PENGENALAN

(BACKGROUND)

Anugerah Industri Koko (AIK) adalah satu pengiktirafan Lembaga Koko Malaysia (LKM) di bawah Kementerian Perusahaan Perladangan dan Komoditi, kepada syarikat-syarikat yang menjalankan aktiviti pembuatan dan pengilangan produk-produk berasaskan koko khasnya produk coklat, kek dan kokies dengan mencapai kualiti, pematuhan undang-undang dan nilai yang ditetapkan bagi kriteria-kriteria kategori-kategori yang dipertandingkan.

Hasrat AIK adalah untuk menggalakkan perkembangan dan pertumbuhan serta mewujudkan persaingan yang sihat di kalangan pengeluar produk makanan berasaskan koko terutama coklat buatan tangan ke arah peningkatan produktiviti, kualiti produk-produk koko dan berdaya saing industri koko.

(Cocoa Industry Awards (CIA) is an acknowledgement by Malaysian Cocoa Board (MCB), an agency under the Ministry of Plantation Industries and Commodities, to companies involve in the manufacturing of cocoa based products of high quality, compliance with the law and values that meet the criterion specified for each category of the competition.

The objective of CIA is to encourage expansion and development as well as to create healthy competition among the entrepreneur of cocoa based products manufacturers in increasing productivity, qualities of cocoa products and competitiveness in the cocoa industry).

TATACARA PERMOHONAN

(APPLICATION PROCEDURES)

Maklumat lisan, dokumen, gambar dan apa-apa bentuk bahan yang diperolehi daripada tuan merupakan maklumat yang akan digunakan untuk AIK. Tatacara penyertaan seperti berikut :

(Information in the form of oral, documents, site visit or in any other form provided by the applicants will be used to verify the claim. Application procedures are as follow);

1. Permohonan ini terbuka kepada syarikat-syarikat usahawan industri kecil dan sederhana (IKS) di Malaysia yang menjalankan aktiviti perniagaan pembuatan produk-produk berasaskan koko, mempunyai nilai jualan tahunan tidak melebihi RM2.5 juta, mempunyai tidak lebih daripada 50 pekerja tetap dan syarikat dimiliki sepenuhnya oleh warganegara Malaysia.

(This competition is opened to small enterpreneurs in Malaysia who is engaged in manufacturing of cocoa products, having annual sales turnover not exceeding RM2.5 million, having not more than 50 full-time employees and equity of the company is fully owned by Malaysian).

2. Pemohon adalah syarikat-syarikat usahawan yang berdaftar sebagai usahawan bimbingan LKM serta menjadi ahli berdaftar dalam Persatuan Usahawan Coklat Buatan Malaysia.

(Applicants are entrepreneurs registered as entrepreneurs under MCB guidance, and registered under Malaysian Handmade Chocolate Association).

3. Permohonan untuk menyertai pertandingan ini hendaklah menggunakan borang yang ditetapkan oleh LKM.

(Application to join this competition must be made using form specified by MCB).

4. Semua maklumat di dalam borang mesti diisi oleh pemohon atau wakilnya dengan betul, jujur dan lengkap.

(Information must be filled correctly, true and complete by applicant).

5. Maklumat perniagaan untuk tempoh tiga (3) tahun iaitu 2007, 2008 dan 2009 adalah digunakan untuk pertandingan ini sahaja.

(Business information for the past three (3) years for 2007, 2008, 2009 will be used for this competition).

6. Maklumat yang diberi perlu dibuktikan dengan dokumen-dokumen sokongan seperti inbois belian, penyata urusniaga, rekod-rekod urusniaga secara bertulis atau bahan bercetak komputer dan penyata pendapatan yang telah diaudit.

(All information must be supported by proper documentation such as purchasing invoice, statement of account, business records in writing or computer generated document and audited income statement).

7. Panel Pemeriksa atau panel penilai AIK boleh meminta apa-apa maklumat tambahan atau memeriksa maklumat-maklumat pemohon untuk mengesahkan maklumat pada borang penyertaan AIK.

(The CIA Evaluation Panel could request any additional information or examine applicant information to verify any information given in the CIA application form).

8. Keputusan panel juri Anugerah Industri Koko adalah muktamad dan tiada bantahan atau keraguan yang boleh dicabar oleh pemohon atau mana-mana pihak.

(The results of the judges of the Cocoa Industry Awards are final and complaints from applicants or other parties would not be entertained).
