Kepada Semua Peserta dan Pasukan PASPOT KUDA / VAKSINASI
Untuk makluman semua, kesemua kuda dan poni yang bertanding di pertandingan ini di wajibkan mengambil vaksin seperti berikut:
a. Equine Influenza (EI) b. Tetanus c. Japanese B Encephalitis (Jap B)
Vaksinasi dan dinyah-cacing, mesti diberi sekurang-kurangnya 7 hari sebelum perjalanan (kemas kini yang terkini dalam passport WAJIB pada 21 Nov 2018 dan bukan selepas tarikh ini). Kuda dan poni perlu diperiksa oleh veterinar masing-masing dan spesifikasi yang sesuai, sebelum perjalanan ke Pertunjukan Kuda Kebangsaan 2018. Mana-mana kuda yang tidak dalam keadaan tenang semasa pemeriksaan Doktor veterinar, kuda-kuda ini tidak di benarkan bertanding. Bagi kuda yang telah di vaksin wajib menunjukkan bukti yang menyatakan kuda tersebut telah di vaksin kepada Pegawai Veterinar Bertugas pada pertandingan tersebut. Segala rekod haruslah di catatkan di dalam kad pendaftaran/paspot kuda yang di keluarkan oleh EAM atau FEI semasa mengisi borang penyertaan. Kuda dan poni yang tidak mempunyai rekod vaksin terkini tidak di benarkan memasuki permis kelab dan pertandingan. Sila pastikan semua dokumen di sediakan. Penyertaan

Tarikh Tutup Penyertaan
Semua penyertaan mesti dibuat menggunakan borang yang ditetapkan. Penyertaan haruslah sampai ke Alamat Penganjur di atas atau email ke [email protected] pada 12hb November 2018 (Isnin). Penyertaan yang diterima selepas tarikh di atas akan dianggap sebagai Penyertaan Lewat. Penyertaan boleh dibuat secara peribadi, melalui pos, faks atau melalui internet dan akan disahkan apabila bayaran penuh diterima. Semua penyertaan yang diterima selepas tarikh tutup akan dikenakan tambahan 50% kecuali dinyatakan dalam jadual.
Borang Penyertaan Semua penyertaan hendaklah di isi di dalam borang penyertaan rasmi dan disertai dengan pembayaran penuh. Sila rujuk Lampiran 3 dan Lampiran 4. Sila isi Borang Penyertaan, Borang Permohonan Stable dan Borang Pengecualian (Waiver Form) dan kembalikan ke alamat di atas sebelum 12hb November 2018 (Isnin).
Pembayaran Penyertaan
Pihak penganjur berhak untuk tidak membenarkan bertanding sekiranya kegagalan mengemukakan semua yuran penyertaan tidak dijelaskan terlebih dahulu. Kesemua pembayaran adalah dalam bentuk tunai sahaja.

Terma dan Syarat Peraturan Pertandingan
Peraturan Pertandingan Pertandingan ini akan dijalankan dibawah peraturan Persatuan Equestrian Australia dan Equestrian Malaysia (EAM). Semua peserta yang menerima hadiah hendaklah berada di atas kuda semasa menerima hadiah.
Upacara Penyampaian Hadiah
Pemenang yang tidak menghadiri upacara penyampaian hadiah tidak akan menerima hadiah mereka. Semua peserta yang menerima hadiah hendaklah berada di atas kuda ketika menerima hadiah.
Peraturan Dalaman Semua peserta dikehendaki memastikan memakai topi, kasut dan pakaian menunggang sepanjang masa ketika berada di atas kuda. Kawasan pertandingan dan kawasan stable mestilah bersih dan rapi pada setiap masa. Memandikan kuda haruslah dilakukan di kawasan mandian kuda (washing bay) yang telah disediakan dan bukan di dalam kandang kuda. Anda akan dibekalkan dengan beg plastik untuk pembuangan kotoran dan sampah sarap. Kotoran / sisa mesti dikeluarkan dari habuk papan (sawdust) setiap hari dan dilupuskan dengan cara yang betul. Penyewaan stable sementara adalah terhad dan akan diberi atas dasar sesiapa yang membuat tempahan terdahulu (first come first serve basis). Kesemua stable hendaklah sentiasa dalam keadaan bersih. Sekiranya terdapat kekotoran, pihak penganjur tidak akan teragak-agak untuk mengenakan bayaran sebagai denda kepada pihak yang terlibat.
Pihak Penganjur Berhak Untuk
Mengenakan bayaran denda kepada pihak yang terlibat melakukan kekotoran dan kerosakan di kawasan pertandingan dan stable. Mengubah waktu pertandingan jika perlu. Membatalkan apa jua acara yang dipertandingkan. Membahagi apa jua acara kepada kumpulan. Menolak sebarang penyertaan atas sebab-sebab tertentu. Menaik dan menurunkan ketinggian halangan (jumping) bergantung kepada penyertaan.

Kebajikan Kuda SETIAP KUDA HANYA DIBENARKAN MAKSIMA BERTANDING SEBANYAK TIGA (3) ACARA SEHARI.

Sistem Pembahagian Kategori
Permohonan pembahagian kategori atas / bawah mestilah dibuat kepada pihak penganjur sebelum tarikh tutup penyertaan.
Stable Sementara RM80.00 (tiada caj tambahan) bagi seekor kuda untuk satu (1) hari. Peruntukan stable akan diberi kepada sesiapa yang membuat pembayaran awal (first come first serve basis).
Borang Pengecualian dan Pengakuan (Waiver Form & Acknowledgement Form)
Sila kemukakan semua borang pengecualian dan pengakuan sebelum tarikh tutup penyertaan. Peserta yang menandatangani borang tidak dibenarkan untuk bertanding. Sila rujuk Lampiran 3 untuk Borang Pengecualian dan Borang Pengakuan.
Kawalan Perubatan Paramedik
Penganjur tidak membenarkan
Sekiranya pihak penganjur mendapati kuda menggunakan Equine Anti- Doping, pihak penganjur berhak untuk tidak membenarkan peserta dan kuda untuk bertanding dengan serta-merta.
Nombor Kuda Nombor kuda; Nombor Pengenalan Kuda (Nombor Bridle) akan diagihkan selepas taklimat pertandingan. Pihak penganjur akan memberi nombor kepada setiap peserta

SILA BACA DENGAN TELITI SEBELUM MENANDATANGANI BORANG INI. BORANG INI HENDAKLAH DITANDATANGANI SEBELUM ANDA MULA BERTANDING.
Setelah dipertimbangkan oleh Jawatankuasa Penganjur Pertandingan Kuda Kebangsaan Kelantan 2018 yang memberikan saya kebenaran untuk mengambil bahagian dalam acara kelas-kelas yang dipertandingkan yang akan diadakan di Royal Kelantan Endurance Stable pada 29/11/2018 sehingga 2/12/2018 bersetuju: -
A. Pengumuman Liabiliti dan Pengecualian
1. Bahawa sebelum menyertai sebarang acara, saya akan memeriksa kemudahan dan peralatan yang akan digunakan oleh saya semasa acara itu dan bersetuju bahawa kemudahan dan peralatan berada dalam keadaan baik.
2. Saya faham bahawa saya mungkin terlibat dalam aktiviti yang melibatkan risiko kecederaan serius, termasuk hilang upaya kekal dan kematian, dan sosial dan mengalami kerugian ekonomi yang teruk yang mungkin disebabkan oleh tindakan saya, dan juga dari tindakan kecuaian yang disengajakan oleh pihak lain, dalam peraturan Pertandingan serta permohonan, dari keadaan premis dan / atau peralatan, dan oleh sebab-sebab lain yang tidak diketahui atau tidak dapat dijangka: dan
3. Untuk melepaskan, membebaskan dan memelihara tanpa membahayakan pihak penganjur dan jawatankuasa mereka, pengarah, ejen, pekerja, rakan sekerja, kakitangan dan kontraktor bebas dari sebarang tuntutan, tindakan, liabiliti dan sebarang punca tindakan dimana saya yang kini mungkin ada atau mungkin menyebabkan keputusan atau insiden yang berkaitan dengan penyertaan sukarela dalam mana-mana aktiviti sukan atau bukan sukan yang berkaitan dengan Pertandingan.
B. Pengakuan Peraturan dan Syarat. 1. Mematuhi semua peraturan dan syarat penganjur yang berkuat kuasa dari semasa ke semasa; 2. Untuk mengelakkan daripada mengambil minuman beralkohol atau menggunakan apa-apa bahan dengan
cara yang sama yang akan mengurangkan atau menjejaskan tahap prestasi saya semasa penyertaan saya di aktiviti sukan atau pertandingan.
3. Untuk menahan diri dari berkelakuan yang tidak sopan atau kelakuan yang membawa kecurigaan apabila bertanding termasuklah secara fizikal tanpa batasan atau menyerang dengan lisan atau menyalahgunakan mana-mana pegawai, pesaing, penonton, sukarelawan atau mana-mana pihak yang terlibat dalam perlakuan dan pengurusan pertandingan dan apa-apa yang kelakuan yang sedemikian boleh mengakibatkan pengusiran dari pertandingan dan mana-mana tindakan jenayah atau sivil terhadap saya, dan;
4. Untuk di filemkan, difoto atau ditemuramah sebagai peserta dalam pertandingan apabila diminta berbuat demikian oleh mana-mana pegawai atau pegawai penganjur. Saya mengakui bahawa saya melaksanakan "Tanggungan Am Liabiliti" ini dengan bebas dan secara sukarela, dan saya lakukan tidak bergantung kepada sebarang dorongan, janji atau perwakilan yang dibuat oleh organisasi atau individu yang terlibat di dalam pertandingan, atau mana-mana pihak yang dikaitkan dengan apa-apa cara dengan pertandingan.
