i) Borang ini adalah sah untuk tempahan penggunaan ruang, peralatan dan perabot di
bawah penyeliaan Unit Pentadbiran, Fakulti Ekonomi dan Perniagaan
ii) Permohonan ruang hendaklah ditempah dalam tempoh empat belas (14) hari bekerja
sebelum tarikh penggunaan.
iii) Pelan susun atur ruang, peralatan dan perabot serta kelulusan pelaksanaan

iv) Borang yang telah diisi lengkap hendaklah diserahkan dalam waktu pejabat.

TERMA & SYARAT
1. Penggunaan ruang, peralatan dan perabot dibenarkan dalam waktu pejabat
sahaja. Keperluan di luar waktu pejabat hendaklah diselaraskan melalui
pegawai/pensyarah selaku penyelaras program/aktiviti;
2. Penggunaan ruang, peralatan dan perabot berkenaan hanya untuk kegunaan
sebagaimana maksud ianya dipohon;
3. Tuntutan kerja lebih masa bagi staf FEB yang bertugas di luar waktu pejabat jika
berkaitan akan ditanggung sepenuhnya oleh pemohon;
4. Staf FEB yang bertugas berhak menamatkan penggunaan ruang, peralatan dan
perabot sekiranya pengguna gagal atau tidak mematuhi arahan yang telah
ditetapkan;
5. Pemohon hendaklah menjaga keselamatan ruang, peralatan dan perabot sepanjang
tempoh penggunaan. Jika berlaku sebarang kerosakan pada mana-mana ruang dan
kehilangan pada peralatan dan perabot FEB akibat kecuaian, kelalaian, kesilapan
dan tinggalan yang berlaku semasa mana-mana ruang digunakan adalah di bawah
tanggungjawab pemohon. Pemohon dikehendaki membaikpulih atau mengganti
alatan tersebut atas kos sendiri;
6. Pemohon memberikan jaminan bahawa FEB akan dikecualikan daripada sebarang
tuntutan daripada pihak ketiga atau pihak luar akibat kecuaian, kelalaian,
kesilapan dan tinggalan oleh pemohon.
7. Selepas penggunaan ruang, peralatan dan perabot, pemohon wajib menyusun dan
mengemas semula seperti keadaan asal sebelum penggunaannya;
8. Sebarang pindaan atau perubahan kepada terma dan syarat ini akan dimaklumkan
dari semasa ke semasa.
