design of a centre gun to be used in lieu of main burners during idling; ­ Explosion of an Air Separation Unit at Shell MDS, possibly due to the infiltration

of haze particles in huge quntities which came into contact with 30 tons of pure oxygen;

­ Explosion in a ship under repair at MSE Pasir Gudang, due to the infiltration of combustible gas from the Crude Oil Washing Line through a worn out drain valve, killing 9 people.
