★ STeriparatide

Myopathy (first report) in an elderly patient: casereport

An 80-year-old woman who received teriparatide developedmyopathy. She was hospitalised with a 1-year history ofmuscle cramps and lower limb weakness. She had beenasymptomatic before starting teriparatide therapy [route, dosage, indication and duration of treatment to reaction onsetnot stated]. Neurological examination revealed mild weaknessof her gluteal muscles. Electromyographic (EMG) examinationrevealed only early recruitment in her gluteus maximus andmedius muscles. Muscle MRI revealed focal areas of fattyreplacement in her gluteus muscles. Teriparatide waswithdrawn. After 3 months, her symptoms had resolved. Herclinical examination and EMG results were normal at follow-up.

Author comment: "In our case, the evidence of minimalmyopathic features on EMG examination, the presence offatty replacement documented by muscle MRI, the temporallink between teriparatide starting and symptom onset, andsymptom resolution after drug discontinuation support thehypothesis that this is a drug-induced myopathy."Luigetti M, et al.
