Reactions 1455 - 8 Jun 2013 S Teriparatide Myopathy (first report) in an elderly patient: case report An 80-year-old woman who received teriparatide developed myopathy. She was hospitalised with a 1-year history of muscle cramps and lower limb weakness. She had been asymptomatic before starting teriparatide therapy [route, dosage, indication and duration of treatment to reaction onset not stated]. Neurological examination revealed mild weakness of her gluteal muscles. Electromyographic (EMG) examination revealed only early recruitment in her gluteus maximus and medius muscles. Muscle MRI revealed focal areas of fatty replacement in her gluteus muscles. Teriparatide was withdrawn. After 3 months, her symptoms had resolved. Her clinical examination and EMG results were normal at follow- up. Author comment: "In our case, the evidence of minimal myopathic features on EMG examination, the presence of fatty replacement documented by muscle MRI, the temporal link between teriparatide starting and symptom onset, and symptom resolution after drug discontinuation support the hypothesis that this is a drug-induced myopathy."
