INSULINE LIKE GROWTH FACTOR-1 PADA PENDERITA
OBESITAS ABDOMINAL
Abstrak
Obesitas sentral (Ob-sen) berhubungan dengan kondisi
resistensi insulin yang mengakibatkan hiperinsulinemia. Beberapa
studi menunjukkan jika kejadian hiperplasia prostat lebih tinggi
pada Ob-sen. Peran resistensi insulin, hubungannya terhadap IGF-
1 perlu dibuktikan lebih lanjut. Penelitian ini bertujuan untuk
membuktikan jika Ob-sen dengan HOMA-IR, IGF-1, IL-6, hsCRP
yang tinggi lebih berisiko mengalami hiperplasia prostat dan
membuktikan lebih lanjut peran resistensi insulin dan IGF-1
sebagai faktor risiko hiperplasia prostat.
Penelitian ini menggunakan rancangan case control dengan
total sampel 80 orang, 40 orang laki-laki Ob-sen dengan
hiperplasia prostat sebagai kasus dan 40 orang Ob-sen tanpa
hiperplasia prostat sebagai kontrol dengan rerata usia 62 tahun.
Analisis data dengan uji independent sampel t-test, chi square dan
uji multivariate regresi logistik. Pemeriksaan IGF-1 dengan metode
Immulite 2000 IGF-1 dengan teknik chemiluminescent
immunometri assay, Pemeriksaan IL-6 dengan tehnik sandwich
enzyme immunoassay dengan Quantikine immunoassay kit buatan
R&D system Inc USA, dan pemeriksaan hsCRP dengan tehnik
immonometric assay.
Glukosa puasa (Z=-3,325, p=0,001), Insulin puasan (Z=-2,15,
p=0,034), HOMA-IR (Z=-2,843, p=0,004), dan IGF-1 (t=3,27,
p=0,002) lebih tinggi dan bermakna pada kelompok kasus

dibandingkan kontrol. hsCRP dan IL-6 tidak berbeda bermakna
antar dua kelompok tersebut. Dari hasil uji bivariate chi-square
didapatkan hanya komponen resistensi insulin (HOMA-IR)
(OR=1,68, p=0,019) dan IGF-1 (OR=2,14, p=0,001) yang
memiliki hubungan dengan hiperplasia prostat pada Ob-sen dan
bermakna secara statistik. Untuk melihat pengaruh secara simultan
antara resistensi insulin dan IGF-1 sebagai faktor risiko terhadap
hiperplasia prostat dilakukan uji multivariate regresi logistik. Dari
hasil uji regresi logistik didapatkan jika resistensi insulin/HOMA-
IR (OR:2,59 IK(0,88-7,02), p=0,043) dan IGF-1 (OR:4,76, IK
(1,71-13,11), p=0,003) terbukti dan bermakna secara statistik
meningkatkan risiko hiperplasia prostat pada Ob-sen.
Dari hasil penelitian ini disimpulkan Ob-sen dengan HOMA-
IR tinggi lebih berisiko terkena hiperplasia prostat dibandingkan
dengan HOMA-IR rendah; Ob-sen sentral dengan kadar IGF-1
tinggi lebih berisiko terkena hiperplasia prostat dibandingkan
dengan kadar IGF-1 rendah; resistensi Insulin yang diukur dari
nilai HOMA-IR dan kadar IGF-1 yang tinggi berperan sebagai
faktor risiko hiperplasia prostat pada obesitas sentral.

FACTOR FOR PROSTAT HYPERPLASIA VIA INSULIN
LIKE GROWTH FACTOR-1 IN ABDOMINAL OBESITY
Abstract
Abdominal obesity usually associate with condition of insulin
resistance and hyperinsulinemia. Numerous studies have found that
prevalence of benign prostatic hyperplasia (BPH) was higher in
abdominal obesity. This study was aimed to evaluated that
abdominal obesity patient with high level of HOMA-IR, IGF-1,
IL-6 and hsCRP have higher risk of BPH, compared
thanAbdominal obesity patients with low level of HOMA-IR, IGF-
1, IL-6 and hsCRP, and to investigate wether insulin resistance and
high level of IGF-1, IL-6 and hsCRP as risk factor of BPH in
Abdominal obesity patients.
The study design wascase control. Eighty patients were
involved in this study, fourthly male central obesity subject with
Prostate hyperplasia as case group and fourthly male Abdominal
obesity without prostatic BPH, with age more than 40 years old
both case and control. Data analysis were done by student t test,
chi square, multivariate logistic regression analysis and completed
by path analysis to measure the effect of each risk factor.
Measurementof IGF-1 with Immulite 2000 IGF-1method with
chemiluminescent immunometri assay, IL-6 was measured with
sandwich enzyme immunoassay with Quantikine immunoassay kit,
production of R&D system Inc USA, and hsCRP was measured
with immonometric assay.
Fasting blood glucose (Z=-3,325, p=0,001), Fasting insulin
level (Z=-2,15, p=0,034), HOMA-IR (Z=-2,843, p=0,004), and

IGF-1 (t=3,27, p=0,002) of case group was higher and statisticaly
significant compared to control group. hsCRP and IL-6 not
significantly different between case and control groups. Analysis
bivariatechi-square test, found that the insulin resistance (HOMA-
IR) (OR=1,68, p=0,019) and IGF-1 (OR=2,14, p=0,001) were
associate with Prostatic hyperplasia in Abdominal obesity patients.
Finally we found that insulin resistance/HOMA-IR (OR:2,59
IK(0,88-7,02), p=0,043) and high level of IGF-1 (OR:4,76, IK
(1,71-13,11), p=0,003) as a risk factors of BPH in Abdomin
obesity patients. Path analysis show us that significantly
correlation between HOMA-IR with IGF-1 (CR=4,61; p<0,001),
HOMA-IR with prostate hyperplasia (CR3,64; p<0,001), IGF1
with prostate hyperplasia (CR=3,19; p<0,001)
It was concluded that HOMA-IR (insulin resistance) and high
level of IGF-1 as risk factors of Prostate hyperplasiain Abdominal
obesity.

1. Pendahuluan
Pandemi obesitas khususnya obesitas abdominal (Ob-Ab) kini
menjadi ancaman serius bagi umat manusia di dunia. Ob-Ab sering
didapatkan secara bersamaan dengan beberapa gejala dan faktor
risiko lainnya yang dianggap sebagai faktor risiko penyakit kardio
vaskuler (PKV) yang dewasa ini dikenal sebagai sindroma
metabolik (SM) (Carr et al., 2004; IDF, 2013). SM merupakan
suatu konstelasi faktor risiko yang terdiri dari resistensi insulin,
Ob-Ab, hipertensi, kegagalan toleransi glukosa (prediabetes) dan
dislipidemia yang dianggap sebagai tahapan awal dalam
perjalanan klinis DM dan Penyakit Kardiovaskuler (PKV) (Leahy,
2005; Grundy et al., 2008; Gorbanchyski et al., 2010). Dalam
perjalanan klinisnya SM sebagai faktor risiko gangguan metabolik
dan PKV, Ob-Ab memegang peran yang paling mendasar (Carr et
al., 2004; Grundy et al., 2008).
Sejak tahun 1980 pandemi obesitas menjadi fakta yang
mendapat perhatian serius dari WHO. Pada tahun 2008 data
terbaru WHO menunjukkan jika lebih dari 1,4 miliar jiwa
penduduk dunia dalam kondisi kelebihan berat badan, dari jumlah
tersebut 200 jiwa pria dan 300 juta perempuan dengan obesitas
(WHO, 2014). Secara keseluruhan lebih dari 10% populasi dunia
dalam kondisi obesitas. Di Amerika Serikat 30% penduduk usia
dewasa didapatkan dengan kelebihan berat badan (BMI 25-29,9
kg/m2) dan 32% dengan obesitas (BMI 30 kg/m
2) (Ogden et al.,
2006). Pada usia diatas 60 tahun prevalensinya bahkan mencapai
40%. Di Asia SM didapatkan paling tinggi di India akibat

tingginya prevalensi Ob-Ab, mencapai 1.091 jiwa dengan
prevalensi 8% pada laki-laki dan 18% pada perempuan (Johnson,
2013).
Prevalensi DM juga terus meningkat dari tahun ke tahun
seiring dengan meningkatnya prevalensi SM dan obesitas
khususnya Ob-Ab. Di tahun 2012 International Diabetes
Federation (IDF) memperkirakan 371 juta jiwa penduduk dunia
(8,3%) menderita DM (IDF, 2012). Dari jumlah tersebut 50%nya
merupakan kasus yang tidak terdiagnosis. Indonesia menempati
peringkat 7 dalam jajaran 10 besar negara dengan jumlah penderita
DM terbesar di dunia (IDF, 2012). Jumlah penderita DM
diperkirakan sekitar 7,3 juta jiwa pada tahun 2011 (IDF, 2011).
Pada tahun 2012 jumlah penderita DM meningkat menjadi 7,6 juta
jiwa (IDF, 2012). Jumlah ini diprediksi akan terus meningkat
hingga mencapai 11,8 juta jiwa pada tahun 2030. Pada saat itu
Indonesia diperkirakan berada pada peringkat ke 9 negara dengan
penderita DM terbesar di dunia. Hasil RISKESDAS oleh DepKes
pada tahun 2007, mendapatkan jika prevalensi DM di Indonesia
mencapai 5,7% (Depkes, 2007).
Komplikasi SM dan DM sudah dimulai sejak dini sebelum
diagnosis ditegakkan. Sekitar 50% pada saat diagnosis ditegakkan
sudah menderita komplikasi kronik, 21 % diantaranya mengalami
retinopati, 18% dengan gambaran EKG yang abnormal, dan 14%
dengan gangguan aliran darah ke tungkai dan sisanya berhubungan
dengan keganasan gastrointestinal dan keganasan di bidang urologi
terutama pada laki-laki (IDF 2012; WHO 2014). Komplikasi DM
tersebut mengurangi produktivitas dan harapan hidup pasien
sampai 15 tahun. Di Dunia 4,8 juta jiwa meninggal karena DM
sebagai akibat SM pada tahun 2012. Mortalitas akibat diabetes di
Indonesia cukup tinggi, sekitar 155.465 jiwa penduduk Indonesia
meninggal karena DM (IDF, 2012).
Disamping sebagai konstelasi faktor risiko yang
mengakibatkan DM dan PKV, SM ataupun masing-masing
komponennya terutama Ob-Ab dan resistensi insulin dari beberapa
studi epidemiologi dihubungkan dengan beberapa penyakit
degeneratif seperti hiperplasia prostat (Gorbanchyski et al., 2010;

WHO, 2014). Pada laki-laki, hiperplasia prostat merupakan salah
satu 3 penyakit degeneratif yang paling sering didapatkan seiring
dengan bertambahnya usia (selain hipogonad dan disfungsi ereksi)
yang sangat berhubungan dengan inflamasi kronik yang bersifat
low grade. Lima belas persen laki-laki diatas 40 tahun ditemukan
dengan kelainan prostat dan prostatitis, 20-40%nya dengan Benign
Prostate Hyperplasia (BPH). Pada tahun 2007 terdapat 218.890
kasus baru BPH di Amerika Serikat dengan kematian mencapai
27.050 dan pada tahun 2010, dan insidennya terus meningkat
mencapai 230.000 kasus baru (Braun et al., 2011).
Resistensi insulin didefinisikan sebagai resistensi terhadap
efek metabolik insulin yang berakibat pada insensitivitas jaringan
terhadap insulin (Hawkins & Rossetti, 2005). Efek metabolik
insulin mencakup efek penghambatan terhadap produksi glukosa
endogen, efek stimulasi pada pengambilan glukosa dan sintesis
glikogen pada jaringan, serta menghambat penguraian lemak pada
jaringan adiposa (Ghani, 2006). Tanpa adanya defek pada fungsi
sel beta pankreas, individu mengkompensasi resistensi insulin
dengan peningkatan jumlah sekresi insulin (hiperinsulinemia)
(Masharani et al., 2004).
Ob-Ab sangat berhubungan dengan kondisi resistensi insulin
sebagai dampaknya yang mengakibatkan hiperinsulinemia (Carr &
Brunzell, 2004). Ob-Ab juga sangat berhubungan dengan inflamasi
sistemik kronik low grade akibat peningkatan sitokin proinflamasi
seperti CRP serta status protrombotik seiring dengan peningkatan
kadar plasminogen activator inhibitor-1 (PAI-1) serta stres
oksidatif (Kahn et al., 2005).
Model patogenesis dari resistensi insulin ini terintegrasi antara
hubungan genetik, obesitas, dan faktor lingkungan (Masharani et
al., 2004; Hawkins & Rossetti, 2005; Kahn et al., 2005). Mutasi
genetik yang terjadi berakibat pada defek aksi insulin dan sekresi
insulin. Resistensi insulin pada obesitas berhubungan dengan
faktor-faktor sirkulasi yang dihasilkan oleh adiposit seperti TNF-α
(Tumor Necrotizing Faktor alfa), FFA (Free Fatty Acid), dan
leptin. Namun yang paling dominan adalah Asam Lemak
Bebas/FFA (Evans et al., 2002). Jaringan lemak intraabdominal

pada obesitas abdominal akan melepaskan FFA berlebihan yang
dapat menginduksi resistensi insulin karena akan bersaing dengan
glukosa sebagai sumber energi untuk dioksidasi di jaringan perifer.
Secara molekuler FFA dapat mengaktivasi protein kinase C
(PKC), nuclear factor kappa Beta (NF-қB), mitogen activated
protein kinase (p38MAPK), NH2-termial jun kinase/Stress
activated protein kinase (JNK/SAPK), yang mengakibatkan
peningkatan fosforilasi serine/threonine pada IR atau substratnya
sebagai penyebab penurunan sensitivitas reseptor terhadap insulin
sehingga dikompensasi dengan kondisi hiperinsulinemia. Jalur-
jalur pensinyalan ini juga berperan dalam meningkatkan produksi
reactve oxygen species (ROS) yang akhirnya menempatkan
penderita SM apalagi yang sudah jatuh ke kondisi DM pada
keadaan stres oksidatif dan kondisi proinflamasi khususnya
inflamasi kronik yang bersifat low grade (Evans et al., 2002; Opie,
2007; Grundy, 2008).
Inflamasi kronik yang bersifat low grade yang terjadi pada
SM akibat meningkatnya inflitrasi makrofag ke jaringan lemak
yang ditandai dengan meningkatnya sitokin proinflamasi seperti
TNF-, IL-6 dan CRP selain berperan dalam patogenesis resistensi
insulin juga dianggap memiliki peran dalam kejadian hiperplasia
prostat (Hsing et al., 2007). Inflamasi pada SM yang dikaitkan
dengan peningkatan kejadian hiperplasia prostat dihipotesiskan
melalui: (1) meningkatnya mediator proinflamasi seperti sitokin
dan ROS; (2) meningkatnya ekspresi onkogen seperti COX-2 dan
MMP; (3) meningkatnya faktor transkripsi gen proinflamsi yaitu
NF-kB, STAT3 dan HIF- (Bruton et al., 2010; Ramos, 2013).
Apabila proses inflamasi kronik, ROS dan serangkaian proses yang
sudah disebutkan diatas mengenai organ target prostat dapat
menimbulkan fokal atropi jaringan prostat yang disertai proliferasi
jaringan epitel dan infiltrasi sel-sel radang yang disebut sebagai
PAI (prolifereative inflammatory atrophy). PIA akan mengalami
transformasi menjadi high grade prostatic hyperplasia (PIN)
(Lehrer 2005; Patel et al., 2013). Kerusakan jaringan dan
serangkaian proses penyembuhan kronik sebagai kompensasi

penyembuhan cedera sel yang berulang juga akan mempercepat
timbulnya nodul BPH (Briganti et al., 2009; Nunzio et al., 2011).
Kondisi hiperinsulinemia sekunder akibat resistensi insulin
pada SM juga berhubungan dengan pembesaran prostat dan
manifestasi klinis lower urinary tract symptoms (LUTS).
Hiperinsulinemia berhubungan dengan peningkatan aktivitas saraf
simpatis dan akan meningkatkan tonus saraf simpatis prostat
sehingga akan memperparah gejala LUTS dalam hubungannya
dengan hiperplasia prostat. Hiperglikemia sendiri juga akan
meningkatkan cytosolic-free calcium pada sel otot polos dan
jaringan neural yang akan meningkatkan aktivasi saraf simpatis.
Hiperinsulinemia juga akan meningkatkan aktivitas aksis
insulin grwoth factor-1 (IGF-1 axis), dimana hiperinsulinemia
akan menurunkan produksi insulin grwoth factor binding protein
1dan 2 (IGFBP-1 dan 2), yang berfungsi mengikat IGFs dan
menurunkan aktivitas biologisnya pada IGF-1 Receptor (IGF-1R)
sehingga akan meningkatkan IGFs bebas yang dapat meningkatkan
proliferasi dan menghambat apoptosis sel (Reneham et al., 2006).
Ikatan IGF-1 atau 2 pada IGF-1R akan mengaktivasi jalur tyrosine
kinase. Posforilasi Shc akan mengaktivasi MAPK yang
berhubungan dengan proliferasi dan hiperplasia termasuk
peningkatan risiko keganasan. Hal ini dapat mempercepat
hiperplasia seluler dalam kaitannya dengan abnormalitas
metabolik, seperti pada hiperplasia prostat (Hsing et al., 2007).
Penelitian mengenai hubungan resistensi insulin yang dinilai
dari HOMA-IR, peran inflamasi yang dinilai dari kadar CRP
terhadap IGF-1 dalam kaitannya dengan peningkatan risiko
hiperplasia prostat pada SM belum pernah dilaporkan.

3. Kerangka Berpikir, Konsep Dan Hipotesis Penelitian
3.1 Kerangka Berpikir
SM merupakan suatu konstelasi faktor risiko yang terdiri dari
resistensi insulin, obesitas abdominal, hipertensi, kegagalan
toleransi glukosa (prediabetes) dan dislipidemia dianggap sebagai
tahapan awal dalam perjalanan klinis DM dan PKV yang
menyebabkan penderita dalam kondisi proinflamasi dan
protrombosis. Disamping sebagai konstelasi faktor risiko yang
mengakibatkan DM dan PKV, SM ataupun masing-masing
komponennya terutama Ob-Ab dan resistensi insulin dari beberapa
studi epidemiologi dihubungkan dengan tingginya angka kejadian
penyakit degeneratif di bidang urologi seperti hiperplasia prostat.
Inflamasi kronik yang bersifat low grade yang terjadi pada
Ob-Ab akibat meningkatnya inflitrasi makrofag ke jaringan lemak
yang ditandai dengan meningkatnya sitokin proinflamasi seperti
TNF-, IL-6 dan CRP selain berperan dalam patogenesis resistensi
insulin juga dianggap memiliki peran dalam kejadian hiperplasia
prostat. Inflamasi pada Ob-Ab yang dikaitkan dengan peningkatan
kejadian hiperplasia prostat dihipotesiskan melalui: (1)
meningkatnya mediator proinflamasi seperti sitokin dan ROS; (2)
meningkatnya ekspresi onkogen seperti COX-2 dan MMP; (3)
meningkatnya faktor transkripsi gen proinflamsi yaitu NF-kB,
STAT3 dan HIF-. Apabila proses inflamasi kronik, ROS dan
serangkaian proses yang sudah disebutkan diatas mengenai organ
target prostat dapat menimbulkan fokal atropi jaringan prostat yang
disertai proliferasi jaringan epitel dan infiltrasi sel-sel radang yang
disebut sebagai PAI (prolifereative inflammatory atrophy). PIA
akan mengalami transformasi menjadi high grade prostatic
hyperplasia (PIN). Kerusakan jaringan dan serangkaian proses
penyembuhan kronik sebagai kompensasi penyembuhan cedera sel
yang berulang juga akan mempercepat timbulnya nodul BPH.
Hiperinsulinemia juga akan meningkatkan aktivitas aksis
insulin grwoth factor-1 (IGF-1 axis), dimana hiperinsulinemia

proliferasi dan menghambat apoptosis sel. Ikatan IGF-1 atau 2
pada IGF-1R akan mengaktivasi jalur tyrosine kinase. Posforilasi
Shc akan mengaktivasi MAPK yang berhubungan dengan
proliferasi dan hiperplasia termasuk peningkatan risiko keganasan.
Hal ini dapat mempercepat hiperplasia seluler dalam kaitannya
dengan abnormalitas metabolik, seperti pada hiperplasia prostat.

3.3 Hipotesis Penelitian
1. Pada obesitas abdominal dengan resistensi insulin
memiliki risiko lebih tinggi terkena hiperplasia prostat
dibandingkan dengan tanpa resistensi insulin.
2. Pada obesitas abdominal dengan kadar IGF-1 tinggi
memiliki risiko lebih tinggi terkena hiperplasia prostat
dibandingkan dengan IGF-1 rendah.
3. Pada obesitas abdominal dengan kadar CRP tinggi
memiliki risiko lebih tinggi terkena hiperplasia prostat
dibandingkan dengan CRP rendah.
4. Pada obesitas abdominal dengan kadar IL-6 tinggi
memiliki risiko lebih tinggi terkena hiperplasia prostat
dibandingkan dengan IL-6 rendah.
4. Metode Penelitian
4.1 Rancangan Penelitian
Penelitian ini menggunakan rancangan penelitian kasus-
kontrol berpasangan (matched-paired case-controle study).
Kelompok kasus adalah pasien obesitas abdominal dengan
hiperplasia prostat dan kontrol adalah obesitas abdominal tanpa
hiperplasia prostat.

4.2 Penentuan Sumber Data
4.2.1 Populasi Penelitian
Populasi target adalah semua penderita Ob-Ab dengan atau
tanpa hiperplasia prostat. Populasi terjangkau adalah semua pesien
dengan Ob-Ab dan hiperplasia prostat yang menjalani pengobatan
di RSUP Sanglah Denpasar dan RSUD Wangaya. Sampel
(intended sample) dipilih dari populasi terjangkau, setelah
memenuhi kriteria inklusi dan eksklusi. Subyek yang benar-benar
diteliti (actual study subjects) adalah sampel yang benar-benar mau
ikut serta dalam penelitian dengan mengisi formulir informed
consent.
4.2.2 Kriteria Inklusi
Laki-laki dengan Ob-Ab dan hiperplasia prostat usia >40
(sebagai kasus) dan sebagai kontrol (usia yang sama) serta
bersedia mengikuti penelitian ini sampai studi ini selesai.
4.2.3 Kriteria Eksklusi
a. Perokok.
b. Keganasan lain (selain hiperplasia / hiperplasia prostat).
c. Sirosis hati.
d. Mendapat obat kortikosteroid.
e. Peminum alkohol.
f. Sedang menderita infeksi akut.
4.2.4 Tehnik Pengambilan Sampel
Pasien yang memenuhi syarat dipilih secara berurutan (secara
consecutive) sampai jumlah sampel yang diperlukan terpenuhi.
Kasus dengan SM dan hiperplasia prostat dipilih dari pasien yang
sudah tegak dengan diagnosis SM dan hiperplasia prostat di RSUP
Sanglah (diagnosis hiperplasis prostat sebelumnya dikonfirmasi
dengan bagian urologi). Kontrol dipilih sesuai dengan karakteristik

masing-masing sampel (orang sehat tanpa SM dan hiperplasia
prostat dengan umur yang sama).
4.2.5 Besarnya Sampel Besar sampel dan kontrol ditentukan dengan rumus untuk studi
kasus kontrol berpasangan (Sastroasmoro, 2002).

90% maka jumlah sampel minimal yang dihasilkan berdasarkan
perhitungan di atas adalah 38 pasangan. Pada penelitian ini
dilibatkan 40 pasangan kasus dan kontrol.

4.5 Lokasi dan Waktu Penelitian Penelitian dilakukan di RSUP Sanglah Denpasar (Pusat
Diabetes dan Poli Urologi) dan RSUD Wangaya. Pemeriksaan lab
rutin dilakukan di Bagian Patologi Klinik RSUP Sanglah, untuk
pemeriksaan lab non-rutin seperti IGF-1 dilakukan di Balai
Pengembangan dan Penelitian Laboratorium Prodia Jakarta.
Penelitian ini dilakukan mulai bulan Maret 2015 sampai dengan
Agusutus 2015.
4.7 Analisis Data
1. Statistik deskriptif.
Untuk data numerik dilakukan analisis deskriptif seperti
rerata±simpangan baku (untuk data berdistribusi normal) median
(minimum-maksimum) (untuk data berdistribusi tidak normal) dan
data nominal disajikan dalam bentuk data frekuensi.
2. Uji normalitas
Untuk data numerik dilakukan uji normalitas kolmogorov-
smirnov, apabila data tersebut tidak berdistribusi normal, maka
untuk dapat dilakukan uji hipotesis dengan asumsi distribusi
normal dapat dilakukan transformasi data menggunakan
transformasi logistik. Data berdistribusi normal bila p>0,05.
3. Uji komparasi
Untuk uji rerata kelompok berpasangan, sebelum uji
komparasinya, dilakukan uji homogenitas variansi kelompok
dengan uji Levene. Uji komparasi dilakukan dengan uji statistik
berdasarkan homogenitas data dari hasil uji Levene. Uji rerata
antar kelompok dilakukan dengan uji parametrik bila data
bersekala numerik dan berdistribusi normal. Uji student’s t
berpasangan melakukan uji hipotesis apakah terdapat perbedaan
rerata antara 2 kelompok. Apabila setelah dilakukan transformasi
data numerik menghasilkan data yang tidak berdistribusi normal,
maka uji hipotesis dilakukan menggunakan analisis non-parametrik
(Man-Withney), dengan sajian data median (interkuartil). Pada uji
bivariate ini juga dilakukan analisis Chi-Square untuk mencari

pola hubungan dan besarnya OR masing-masing variabel
independen.
4. Uji multivariat
Uji multivariat dapat dipakai untuk melihat hubungan antara
satu atau lebih variabel dependen dengan satu atau lebih variabel
independen. Uji ini juga dipakai untuk melihat hubungan
kontributif dari berbagai variabel independen terhadap variabel
dependen, atau melakukan kontrol variabel pengganggu, sehingga
hubungan variabel independen hipotesis dapat dilihat secara murni
terhadap variabel dependen. Analisis regresi logistik dilakukan
untuk melakukan kontrol variabel pengganggu, sehingga pengaruh
variabel independen dapat dilihat secara murni terhadap variabel
dependen yang bersekala nominal. Disajikan Estimated Odd ratio
(OR) dengan konfiden interval 95%.
5. Analisis jalur (path analysis)
` Untuk melihat hubungan langsung antara HOMA-IR, IGF-1,
IL-6 dan hsCRP terhadap hiperplasia prostat serta hubungan tidak
langsung antara HOMA-IR terhadap hiperplasia prostat melalui
peningkatan IGF-1.
6. Tingkat kemaknaan (α) penelitian ini ditetapkan pada nilai
probabilitas (p) kurang dari 0,05. Uji statistik dengan
memakai bantuan perangkat lunak SPSS for Mac versi 21 dan
AMOS versi 23.
5. Hasil Penelitian
5.1 Karakteristik Dasar Subyek Penelitian
Sebanyak 80 sampel laki-laki dilibatkan dalam penelitian ini.
40 sampel sebagai kelompok kasus, dengan hiperplasia prostat dan
40 sampel sebagai kontrol, dengan rerata umur 62 tahun. Setelah
dilakukan uji normalitas data dengan uji kolmogorov-smirnov, dan
setalah dilakukan transformasi logaritma beberapa variabel dalam
penelitian ini memiliki distribusi tidak normal seperti variabel hs-

CRP, IL-6, Insulin puasa, glukosa darah puasa dan 2 jam PP,
HOMA-IR, TG, SC, berat badan, BMI dan lingkar pinggang
(p<0,05). Data karakteristik dasar penelitian ini disajikan pada
Tabel 5.1.

berdistribusi normal, Man-Withney untuk variabel berdistribusi
tidak normal) menunjukkan jika beberapa variabel menunjukkan
perbedaan bermakna antara kelompok kasus dan kontrol. Glukosa
puasa (Z=-3,325, p=0,001), Insulin puasan (Z=-2,15, p=0,034),
HOMA-IR (Z=-2,843, p=0,004), dan IGF-1 (t=3,27, p=0,002)
lebih tinggi dan bermakna pada kelompok kasus (Ob-Ab dengan
hiperplasia prostat) jika dibandingkan kontrol (Ob-Ab tanpa
hiperplasia prostat). Di lain pihak hsCRP dan IL-6 tidak berbeda
bermakna antar dua kelompok tersebut. Sehingga dari analisis
bivariate hanya resistensi insulin dan IGF-1 yang terbukti lebih
tinggi dan bermakna pada Ob-Ab dengan hiperplasia prostat jika
dibandingkan dengan Ob-Ab tanpa hiperplasia prostat.
5.2 Analisis Bivariate Chi-Square Resistensi Insulin (HOMA-
IR), IGF-1, Inflamasi (IL-6, hsCRP) dan Hiperplasia
Prostat
Untuk menilai peran resistensi insulin (HOMA-IR), IGF-1
sebagai faktor risiko hiperplasia prostat dilakukan analisis regresi
logistik untuk mencari nilai OR. Cut off point berdasarkan studi
dan kepustakaan dipakai digunakan untuk membagi variabel
numerik tersebut menjadi variabel dikotom. Kadar IGF-1 normal
untuk populasi normal, dan untuk usia diatas 40 tahun adalah 150
ng/mL (Bravermen et al., 2013). IGF-1 normal (nilai terendah-150
ng/mL) dan IGF-1 tinggi (>150 ng/mL). Untuk cut off point nilai
HOMA-IR, resistensi insulin pada populasi sehat tanpa DM adalah
2,7 (Komshian
et al., 2000). Sehingga nilai HOMA-IR pada penelitian ini dibagi
menjadi resistensi insulin (2,7) dan tanpa resistensi insulin (nilai
terendah-2,6). Di lain pihak untuk marker inflamasi, nilai CRP dan
IL-6 yang dianggap batas atas normal pada populasi dengan risiko
mengalami resistensi insulin adalah berturut-turut 1,32 mg/L dan
12,56 pg/mL (Deepa et al., 2006). Namun pada penelitian ini nilai
IL-6 semua sampel baik pada kasus ataupun kontrol berada dalam
rentang normal (0,15-5,46 pg/mL), sehingga cut-off point yang
dipakai adalah dari median dari hasil penelitian ini yaitu 0,875

pg/mL. Hasil analisis bivariate dengan uji chi-square disajikan
pada Tabel 5.2 sementara hasil uji regresi logistik yang
menggambarkan peran HOMA-IR dan IGF-1 yang tinggi sebagai
faktor risiko hiperplasia prostat pada Ob-Ab disajikan pada Tabel
5.3.
Dari hasil uji bivariate chi-square didapatkan hubungan
bermakna antara resistensi insulin (HOMA-IR) dan IGF-1 dengan
hiperplasia prostat. Apabila ditinjau satu per satu secara bivariate,
HOMA-IR dan IGF-1 masing-masing meningkatkan risiko
hiperplasia prostat. HOMA-IR meningkatkan risiko hiperplasia
prostat dengan OR=1,94 (IK 1,30-2,89) dan bermakna secara
statistik dengan nilai p=0,005. IGF-1 meningkatkan risiko
hiperplasia prostat dengan OR=2,14 (IK 1,38-3,3), p=0,001.
Dari hasil analisis ini juga didapatkan bahwa variabel
inflamasi seperti IL_6 dan hsCRP tidak berhubungan bermakna
dengan hiperplasia prostat. hsCRP dengan nilai OR=0,9; nilai
p=0,653 sedangkan IL-6 nilai OR=1,1105 dengan nilai p=0,655.
Namun terdapat kecenderungan IL-6 menigkatkan hiperplasia
prostat, meskipun tidak bermakna secara statistik.

1 terhadap Hiperplasia Prostat pada Ob-Ab.
Pada analisis multivariate, uji regresi logistik dengan
metode Backward, ketika peran variabel HOMA-IR, IGF-1
diperhitungkan secara simultan, HOMA-IR (resistensi insulin) dan
IGF-1 tetap berperan bermakna meningkatkan risiko hiperplasia

Seperti yang disajikan pada Tabel 5.3, resistensi
insulin/HOMA-IR dan IGF-memiliki pengaruh bermakna terhadap

kejadian hiperplasia prostat pada Ob-Ab dengan OR dan nilai-p
berturut-turut OR=4,18; p=0,03; IK 1,15-15,00 dan OR=4,93; IK
1,76-13,78; p=0,002,. Apabila dianalisis lebih lanjut peran
resistensi insulin (HOMA-IR) dalam memprediksi dan
meningkatkan risiko kejadian hiperplasia prostat pada Ob-Ab
maka didapatkan y=-0,89+1,43(HOMA-IR)+1,59(IGF-1).
Probabilitas untuk kejadian hiperplasia prostat adalah p=1/(1+e-y
),
dimana e adalah bilangan natural bernilai 2,7. HOMA-IR bernilai 1
bila kadarnya>2,7, IGF-1 bernilai 1 bila kadarnya >150ng/mL.
Semua variabel tersebut bernilai 0 pada persamaan y bila nilainya
<nilai cut-off point tersebut. Sehingga apabila didapatkan kadar
IGF-1>150ng/mL dan HOMA-IR>2,7 maka probabilitas untuk
mengalami hiperplasia prostat adalah 89%.
5.4 Analisis Jalur (Path Analysis) Hubungan HOMA-IR, IGF-
1 dan Hiperplasia Prostat
Untuk melihat hubungan kausal efek dari HOMA-IR, IGF-1
terhadap hiperplasia prostat dilakukan analisis jalur (path
analysis). Pada analisis ini variabel eksogen adalah HOMA-IR,
IGF-1, IL-6 dan hsCRP. Sedangkan variabel endogen adalah
hiperplasia prostat. Berdasarkan dari telaah pustaka dan hasil
analisis seperti yang didapatkan di atas, maka dibuatkan model dan
output analisisnya disajikan pada Gambar 5.1.
Pada tahap dari model struktural didapatkan efek langsung
terhadap BPH/LUTS didapatkan sebagai berikut: HOMA-IR
terhadap BPH/LUTS adalah 0,31 (31%), IGF-1 terhadap
BPH/LUTS adalah 0,03 (3%), IL-6 terhadap BPH/LUTS adalah
0,10 (10%) dan hsCRP terhadap BPH/LUTS adalah 0,05 (5%). Di
lain pihak efek langsung HOMA-IR terhadap variabel lain: efek
HOMA-IR terhadap IGF-1, IL-6, hsCRP berturut-turut adalah 3,79
(379%), 0,005 (0,5%) dan 0,015 (1,5%). Dari model struktural ini
tampak bahwa HOMA-IR memiliki efek paling kuat terhadap IGF-
1, sedangkan variabel yang paling kuat pengaruhnya terhadap
BPH/LUTS adalah HOMA-IR.

Hubungan antar konstruk disajikan pada Tabel 5.4 setelah
dianalisis dengan AMOS. Besarnya efek masing-masing variabel
bebas terhadap variabel tergantung dinyatakan dengan critical
ratio (CR). Nilai CR didapatkan dari nilai estimasi dibagi dengan

nilai standard error (SE). Semakin tinggi nilai CR maka efeknya
semakin signifikan.

Dari hasil analisis hubungan antara 2 variabel konstruk ada
Tabel 5.4, dapat diinterpretasikan: terdapat hubungan bermakna
antara HOMA-IR dengan IGF-1 (CR=4,61; p<0,001), HOMA-IR
dengan hiperplasia prostat (CR=3,64; p<0,001), IGF-1 dengan
hiperplasia prostat (CR=3,19; p=0,001). Di lain pihak tidak

didapatkan hubungan bermakna antara IL-6 dan hsCRP terhadap
hiperplasia prostat.
Berdasarkan nilai Standardize regression weight dapat dilihat
seberapa kuat hubungan antar variabel konstruk. Faktor loading
HOMA-IR dan IGF-1 terhadap hiperplasia prostat berturut-turut
0,37 dan 0,32. Hal ini berarti HOMA-IR dan IGF-1 dapat
menjelaskan kejadian BPH, terdapat hubungan erat antara HOMA-
IR dan IGF-1 terhadap hiperplasia prostat. Di lain pihak faktor
loading HOMA-IR terhadap IGF-1 didapatkan sebesar 0,46. Hal
ini menunjukkan hubungan yang erat antara HOMA-IR dan IGF-1.
Pola hubungan antar variabel konstruk, hubungan langsung
ataupun tidak langsung disajikan pada Tabel 5.5 dan 5.6.

Dari tabel 5.5 terlihat dengan jelas HOMA-IR memilki efek
total dan efek langsung paling kuat terhadap hiperplasia prostat.
Sedangkan pada Tabel 5.6 jelas menunjukkan efek total dan efek
langsung yang paling kuat adalah efek HOMA-IR terhadap IGF-1,
diikuti oleh efek HOMA-IR terhadap hiperplasia prostat. Pada
Tabel 5.5 juga terlihat HOMA-IR masih memiliki kontribusi efek
tidak langsung terhadap hipeplasia prostat, sedangkan IGF-1 hanya
memiliki efek langsung terhadap hiperplasia prostat.
Gambar 5.1 dari analisis struktural model dan Tabel 5.6
terlihat bahwa terdapat hubungan yang signifikan dan erat antara
HOMA-IR dengan hiperplasia prostat (CR=3,64; p<0,001, efek
total 44%, efek langsung 31% serta efek tidak langsung 13%). Dari
analisis ini juga didapatkan hubungan yang signifikan antara
HOMA-IR dengan IGF-1 (CR=4,61; p<0,001, efek langsung dan
efek langsung yang sama besarnya yaitu 379%). Terdapat juga
hubungan langsung yang sangat jelas antara IGF-1 terhadap
hiperplasia prostat (CR=3,19; p=0,001, dengan efek langsung 3%).
Dengan demikian dapat dikatakan bahwa HOMA-IR dan IGF-
1 memiliki efek yang signifikan terhadap hiperplasia prostat. Dari
gambar analisis struktural model yang disajikan pada Gambar 5.1
dan analisis jalur di atas, dapat dilihat hubungan tidak langsung
antara HOMA-IR terhadap hiperplasia prostat melalui peningkatan
IGF-1.

6. Pembahasan
6.1 Resistensi Insulin dan Hiperplasia Prostat Pada Ob-Ab
Penelitian ini mendapatkan jika HOMA-IR lebih tinggi dan
bermakna pada kelompok dengan Ob-Ab tanpa hiperplasia prostat
dibandingkan dengan tanpa hiperplasia prostat (1,88 vs 1,19,
p=0,002). HOMA IR yang tinggi >2,7 atau dalam kondisi
resistensi insulin meningkatkan risiko hiperplasia prostat dengan
OR=1,94 (IK 1,30-2,89), dan bermakna (p=0,005). Penelitian ini
konsisten dengan apa yang didapatkan oleh Zhang et al., melalui
studi epidemiologi yang melibatkan 401 sampel laki-laki usia
lanjut juga mendapatkan jika laki-laki dengan SM memiliki
volume prostat yang lebih besar dan bermakna (p=0,000) jika
dibandingkan dengan tanpa SM (Zhang et al., 2014). Sub analisis
lebih lanjut pada penelitian ini didapatkan, hubungan positif
bermakna antara volume prostat dengan BMI (p=0,000), HOMA-
IR (p=0,003) dan berhubungan negatif dengan kadar HDL
(p=0,000). Pada analisis multivariate regresi linier volume prostat
berhubungan secara signifikan dengan HOMA-IR (p=0,015).
Kim et al., yang meneliti 212 pasien BPH tanpa diabetes
mendapatkan hubungan positif bermakna antara kadar glukosa
puasa dengan ukuran prostat (r=0,186; p=0,007) (Kim et al., 2011).
Studi ini juga meneliti hubungan BMI, kadar insulin puasa dan
resistensi insulin terhadap ukuran prostat yang dinilai dari USG
transrektal. Namun pada penelitian ini tidak didapatkan hubungan
yang bermakna antara resistensi insulin (HOMA-IR) dan kadar
insulin puasa dan ukuran prostat. Prevalensi SM pada kelompok
dengan hiperplasia prostat juga didapatkan lebih tinggi jika
dibandingkan tanpa SM (60,3% vs 46,1%, p=0,018) (Ryl et al.,
2015). Penelitian ini menggunakan design case control dengan
kontrol orang sehat. Lebih lanjut pada penelitian ini juga terbukti
hubungan bermakna antara BPH dengan beberapa komponen SM
seperti glukosa darah, kolesterol total, LDL, HDL dan tekanan
darah sistolik dan diastolik. Namun pada penelitian ini tidak
didapatkan hubungan bermakna antara hiperplasia prostat dengan
lingkar pinggang, BB dan konsentrasi TG.

Ozden et al., secara lebih mendetail meneliti hubungan antara
SM dengan penambahan ukuran prostat tiap tahun, dengan
melibatkan 78 pasien dengan gejala hiperplasia prostat dibagi
menjadi kelompok dengan SM dan tanpa SM. (Ozden et al.,
2007). Pada kelompok SM dengan hiperplasia prostat nilai median
BB, BMI, TG dan glukosa darah yang yang lebih tinggi jika
dibandingkan pasien BPH tanpa SM. Pada kelompok SM dengan
hiperplasia prostat pada akhir penelitian ditemukan dengan
penambahan volume prostat total per tahun sebesar 1 ml
sedangkan kelompok BPH tanpa SM hanya 0,93 ml, dengan
perbedaan yang bermakna secara statistik (p<0,05). Dari penelitian
ini digambarkan peran sentral resistensi insulin pada SM dalam
hubungannya dengan hiperplasia prostat.
Konsistensi obesitas sebagai faktor risiko hiperplasia prostat
juga dibuktikan pada studi oleh Lee et al., yang meneliti pengaruh
Obesitas yang diukur dengan parameter BMI terhadap hiperplasia
prostat (Lee et al., 2006). Pada penelitian ini dilibatkan 146 laki-
laki usia >40 tahun yang tidak mengalami DM. Pemeriksaan
ukuran prostat dengan transrektal USG. Subyek penelitian dibagi
menjadi 3 kelompok yaitu normal (BMI 22,9 kg/m2), overweight
(23-24,9 kg/m2) dan obesitas (25 kg/m
2) dan 2 kelompok
berdasarkan lingkar pinggang (>90 cm) dan (90 cm). Volume
prostat didapatkan paling tinggi dan bermakna pada kelompok
obesitas (p=0,03) dan pada kelompok obesitas abdominal
(p=0,002). Setelah dilakukan adjusted terhadap beberapa
confounding variabel, obesitas abdominal menjadi faktor risiko
utama kejadian hiperplasia prostat (volume prostat >20mL)
(OR=3,37, p=0,037).
Hiperinsulinemia yang diinduksi oleh diet tinggi karbohidrat
dan lemak, dapat menyebabkan peningkatan aktivasi sel untuk
berproliferasi secara berlebihan. Studi menunjukkan
hiperinsulinemia berhubungan dengan peningkatan ukuran prostat
sebesar 45% pada percobaan tikus. Hal ini memberikan gambaran
bahwa obesitas dengan hiperinsulinemia mempunyai hubungan
yang kuat dengan kejadian hiperplasia prostat (Renehan et al.,
2008; Allot et al., 2013).

Hiperinsulinemia dan resistensi insulin di lain pihak juga
dapat menigkatkan aktivitas sistem saraf simpatis, dengan
meningkatkan cytosolic-free calcium pada sel otot polos dan
jaringan neural sehingga akan meningkatkan tonus otot polos
prostat sehingga meningkatkan gejala LUTS dan hiperplasia
prostat (Sarma et al., 2009). Jadi obesitas abdominal dengan
HOMA-IR tinggi atau resistensi insulin lebih berisiko mengalami
hiperplasia prostat dibandingkan dengan obesitas abdominal
dengan HOMA-IR rendah atau tanpa resistensi insulin.
Pada penelitian ini, didukung secara tidak langsung oleh hasil-
hasil penelitian lain yang dipaparkan diatas maka Ob-Ab dengan
resistensi insulin memiliki risiko lebih tinggi mengalami
hiperplasia prostat jika dibandingkan dengan obesitas abdominal
tanpa resistensi insulin. Dari sisi hiperplasia prostat, aspek klinis
yang dapat disintesis adalah apabila pasien dengan gejala IPSS ≥8,
gejala LUTS dengan didapatkannya resistensi insulin
meningkatkan risiko hiperplasia prostat maka pada Ob-Ab
perbaikan resistensi insulin, usaha menurunkan LP dapat
dianjurkan lebih dini sehingga risiko hiperplasia prostat bisa
diturunkan.
Resistensi insulin yang mengakibatkan hiperinsulinemia pada
Ob-Ab dalam meningkatkan risiko hiperplasia prostat, apabila
disintesis dari konsep dan teori yang berkembang dapat berikatan
dengan reseptor IGF-1 mengingat insulin memiliki sifat homolog
dengan IGF-1, atau melalui jalur tidak langsung melalui penurunan
IGF-1BP sehingga akan meningkatkan kadar IGF-1 bebas.
Pembuktian mengenai mekanisme lebih lanjut akan dipaparkan
pada sub Bab 6.2 dan 6.4.

6.2 Kadar IGF-1 yang Tinggi dan Hiperplasia Prostat Pada
Ob-Ab
Pada penelitian ini didapatkan kadar IGF-1 yang lebih tinggi
dan bermakna pada kelompok Ob-Ab dengan hiperplasia prostat
jika dibandingkan dengan kelompok Ob-Ab tanpa hiperplasia
prostat (165,4864,62 vs 127,5334,63, p=0,002). Lebih lanjut juga didapatkan jika kadar IGF-1 yang tinggi (>150 ng/mL)
meningkatkan risiko hiperplasia prostat secara bermakna dengan
OR=2,139 (IK=1,34-3,3) dengan nilai p=0,001. Hasil penelitian ini
konsisten dengan beberapa penelitian yang mendapatkan jika IGF-
1 lebih tinggi pada pasien dengan hiperplasia prostat dan obesitas
jika dibandingkan tanpa hiperplasia prostat.
Kadar IGF-1 berhubungan secara bermakna dengan volume
prostat pada kasus-kasus hiperplasia prostat (r=0,596, p=0,007),
namun tidak didapatkan hubungan bermakna pada kasus kanker
prostat (Ayati et al., 2012). Hasil studi ini menggambarkan jika
ada suatu mekanisme yang melibatkan IGF-1 dalam meningkatkan
proliferasi sel-sel prostat namun tidak menyebabkan proses
diferensiasi lebih lanjut. Jalur pensinyalan IGF-1 sangat penting
bagi pertumbuhan, diferensiasi sel, sehingga peningkatan ekspresi
dan kadar IGF-1 berhubungan dengan peningkatan risiko
hiperplasia. Studi pada hewan coba oleh Lefko et al., mendapatkan
jika IGF-1 memberikan efek hiperplasia sel pada prostat dengan
mekanisme yang cukup spesifik (Lefko et al., 2008). Pada
penelitian ini dibuktikan pada mencit transgenik (PB-Des),
ekspresi IGF-1 akan meningkatkan lesi hiperplasia pada prostat,
namun tidak akan jatuh pada kondisi adenocarcinoma.
Pendekatan populasi yang berbeda dipakai oleh Protopsaltis et
al., dalam mengelaborasi pola hubungan sistem IGF dengan
hiperplasia prostat, dilakukan penelitian mengenai hubungan
ekspresi IGFBP-3 pada jaringan prostat dengan hiperplasia prostat
pada populasi pre-diabetes (Protopsaltis et al., 2013). Studi ini
melibatkan 49 sampel dengan pre-diabetes (25 orang dengan IGT,
16 dengan IFG-IGT dan 5 sampel dengan IFG saja). Dari analisis
regresi linier didapatkan IGFB-3 yang tinggi berhubungan
bermakna dengan hiperplasia prostat (p=0,024). Namun tidak

didapatkan hubungan yang bermakna antara ekspresi IGF-1
jaringan dengan hiperplasia prostat. Studi ini tidak dilakukan
dengan design kasus kontrol sehingga tidak bisa menggambarkan
peningkatan risiko hiperplasia prostat yang diakibatkan oleh
peningkatan IGFB-3. Pada penelitian kami, dengan rancangan
kasus kontrol mendapatkan jika kadar IGF-1 yang tinggi
meningkatkan risiko hiperplasia prostat.
Apabila dikaji dan disintesis lebih mendalam, pada prostat
yang sehat, sel stromal memproduksi IGFs, IGFBP-2, IGFBP-3,
IGFBP-4, sedangkan sel epitel dominan mengekspresikan IGF-1R,
IGFBP-2 dan IGFBP-4 serta sebagian kecil IGFBP-3, and IGFBP-
6 (Russel et al., 1998). Pada prostat yang mengalami proliferasi,
sel epitel memproduksi cathepsin D dan PSA yang dapat berperan
menyerupai protease dalam memecah ikatan antara IGFs dengan
IGFBPs sehingga meningkatkan kadar IGFs bebas (Lubik et al.,
2011). Ekspresi yang berlebihan dari IGF-1 dapat menstimulasi
terjadinya pertumbuhan sel, proliferasi sel dan antiapoptosis
melalui mekanisme parakrin maupun autokrin (paracrine or
autocrine mechanism) (Dasgupta et al., 2012).
Insulin mensupresi beberapa IGFBPs dan menstimulasi
produksi IGFs sehingga meningkatkan bioavailabilitas IGFs.
Insulin memiliki struktur yang mirip dengan IGF-1 dan berikatan
dengan reseptor IGF-1 sehingga dapat mengaktivasi berbagai jalur
pensinyalan yang kompleks yang berhubungan dengan proliferasi
sel-sel pada prostat, atau melalui jalur alternatif yang dapat
menurunkan IGF-1BP sehingga meningkatkan bioavalibilitas IGF-
1 (Sarma et al., 2009).
Ikatan antara IGF-1 atau insulin dengan IGF-1R menginduksi
aktivasi jalur tyrosine kinase di dalam sel (Burton et al., 2010).
Fosforilasi dari Shc akan mengaktivasi jalur MAPK yang
menstimulasi pertumbuhan dan proliferasi sel, sedangkan
fosforilasi terhadap IRS akan mengaktivasi jalur PI3K yang
menyebabkan terjadinya hambatan proses apoptosis dan proliferasi
sel melalui stimulasi terhadap beberapa faktor transkripsi seperti
NF-κB (nuclear factor-κB), dan mTOR (mammalian Target Of
Rapamycin). Secara molekuler, aktivasi jalur kinase inilah yang

berperan penting mencetuskan terjadinya hiperplasia prostat
(Rawlands et al., 2012).
Apabila diuji secara bivariate, pada penelitian ini terbukti jika
Ob-Ab dengan kadar IGF-1 yang tinggi (>150 ng/mL) memiliki
risiko lebih tinggi mengalami hiperplasia prostat jika dibandingkan
dengan Ob-Ab dengan kadar IGF-1 rendah.
6.3 Inflamasi (IL-6 dan hsCRP) dan Hiperplasia Prostat Pada
Ob-Ab
Dalam hubungannya dengan inflamasi yang berdasarkan
kajian pustaka dianggap sebagai peran sentral dan variabel antara
yang menjembatani antara resistensi insulin dengan hiperplasia
prostat pada Ob-Ab, pada penelitian ini justru tidak mendapatkan
perebedaan bermakna kadar IL-6 dan hsCRP pada kelompok Ob-
Ab dengan hiperplasia prostat dibandingkan dengan tanpa
hiperplasia prostat. Namun pada penelitian ini didapatkan
kecenderungan kadar IL-6 dan hsCRP sebagai marker inflamasi
lebih tinggi pada kelompok kasus dibandingkan kontrol berturut
turut 1,07 (0,25-5,68) vs 0,71 (0,15-3,62) pg/dL dan 1,45 (0,3-
13,5) vs 1,00 (0,2-9,2) mg/L. Hal ini mungkin disebabkan oleh
rata-rata baik pada kelompok kasus ataupun kontrol memiliki
kadar IL-6 dalam batas normal. Kadar interleukin dan reaktan fase
akut ini kadarnya sangat berfluktuasi pada plasma sehingga tidak
menggambarkan keadaan yang sebenarnya. Di lain pihak kondisi
yang mungkin berpengaruh adalah beberapa sampel dalam
penelitian ini diambil pada kondisi pasien sudah dioperasi,
sehingga beban sitokin akan berkurang dengan telah berkurangnya
masa prostat yang mengalami hiperplasia.
Studi potong lintang oleh Saadi et al., mendapatkan jika
hsCRP dan IL-6 paling tinggi pada kelompok BPH dengan LUTS
jika dibandingkan dengan kelompok BPH saja atau dengan orang
sehat (Saadi et al., 2012). Penelitian ini melibatkan 56 sampel yang
dibagi menjadi 3 kelompok, kelompok BPH+LUTS, kelompok
BPH saja dan orang sehat. Untuk hsCRP, kadar rata-rata pada
masing-masing kelompok tersebut didapatkan 10,04±1,58; 5±0,36;

2,8±0,5 mg/L dengan nilai p<0,001. Untuk IL-6 kadar rata-rata
pada masing-masing kelompok tersebut berturut-turut 14,56±1,1;
8,82±0,93; 12,6±0,82 dengan nilai p<0,001.
Penelitian lain oleh Hung et al., mendapatkan hubungan
bermakna hsCRP dengan LUTS pada pasien BPH, namun tidak
didapatkan hubungan bermakna antara hsCRP dengan volume
prostat (Hung et al., 2014). Studi ini merupakan studi potong
lintang yang cukup besar dengan melibatkan 853 sampel.
Didapatkan kadar rata-rata hsCRP pada karakteristik dasar subyek
sebesar 3,1±0,43 mg/dL, berhubungan secara bermakna dengan
IPSS dengan koefisien korelasi r=0,151, p<0,001.
Apabila kita bandingkan, penelitian kami mendapatkan kadar
IL-6 pada kelompok Ob-Ab dengan hiperplasia prostat sangat
rendah, bahkan dengan rentang antara 0,25-3,72 mg/dL. Hal ini
juga sangat berpengaruh terhadap hasil yang tidak bermakna IL-6
dan hsCRP meningkatkan risiko BPH pada Ob-Ab meskipun
secara konsep dan kajian teori inflamasi memang memegang
peranan sentral dalam hubungannya dengan fibrosis dan
hiperplasia prostat.
Pada penelitian kami hanya didapatkan kecenderungan IL-6
dan hsCRP lebih tinggi pada kelompok kasus jika dibandingkan
kontrol. Kadar IL-6 dan hsCRP pada penelitian kami jauh lebih
rendah jika dibandingkan dengan penelitian-penelitian sebelumnya
seperti yang disebutkan di atas.
Beberapa studi epidemiologis menunjukkan lebih dari 25%
keganasan berhubungan dengan inflamasi kronis dan sebesar 15%
dari kematian pada pasien kanker disebabkan karena inflamasi
(Hsing, 2007; Burton et al., 2007). Pelepasan mediator inflamasi
yang berlangsung kronis dan berlebihan secara teori dianggap
dapat menyebabkan peningkatan inisiasi, promosi dan progresi
tumor (Lehrer et al., 2005; Bruton et al., 2007).
Pada obesitas, jaringan adipose berada dalam kondisi
inflamasi kronis, yang mempunyai peranan yang penting terhadap
timbulnya resistensi insulin, dislipidemia dan diabetes tipe 2,
begitu juga penyakit komorbid lain seperti penyakit kardiovaskuler

dan kanker. Inflamasi ini ditandai dengan peningkatan kadar C-
reactive protein (CRP), interleukin 6 (IL-6), IL-8, monocyte
chemotactic protein-1 (MCP-1), and tumor necrosis factor alpha
(TNF𝛼) (Pickup, 2004; Liang et al., 2007).
Mekanisme yang menerangkan bagaimana inflamasi kronis
yang diinduksi oleh obesitas dapat meningkatkan hiperplasia
prostat meliputi: (i) meningkatnya produksi mediator proinflamasi
seperti sitokin, kemokin, reactive oxygen intermediates (ROI), (ii)
meningkatnya ekspresi onkogen, COX-2 (cyclooxygenase-2), 5-
LOX (5-lipoxygenase), and MMPs (matrix metalloproteinases),
(iii) meningkatnya faktor transkripsi pro inflamasi
(proinflammatory transcription factors) seperti NF-κB (nuclear
factor-κB), STAT3 (signal transducer and activator of
transcription 3), AP-1 (activator protein 1) (Braun et al., 2011;
Ramos-nino, 2013).
Infiltrasi dari inflamasi kronis sering terjadi di daerah perifer
prostat. Inflamasi digambarkan dengan infiltrasi lekosit, pelepasan
sitokin dan kemokin termasuk CRP. CRP juga dilepaskan oleh
jaringan adipose. CRP adalah protein fase akut yang disekresikan
terutama oleh hepar dan merupakan petanda tidak spesifik dari
adanya inflamasi, infeksi dan kerusakan jaringan. Mediator
inflamasi ini selanjutnya mengaktifasi faktor transkripsi
proinflamasi yang lain seperti NF-κB dan inducible nitric oxide
synthase (iNOS) (Ramos-nino, 2013). NF-κB merupakan induser
yang kuat dari gen yang berhubungan dengan aktifitas anti
apoptosis (BCL-XL) dan gen yang mengatur siklus hidup sel
(cyclin D1). NF-κB dan iNOS sangat penting peranannya dalam
aktifasi imun seluler, mempunyai efek mitogenic dan anti
apoptosis. Aktifasi NF-κB dan iNOS oleh sitokin proinflamasi,
akan menginduksi inisiasi sel tumor dan progresifitasnya (Lubik,
2011; Dasgupta et al., 2012).
Sitokin proinflamasi juga akan menginduksi lekosit untuk
memproduksi reactive oxygen (ROS) and nitrogen species (RNS),
yang selanjutnya menginduksi mutasi DNA sehingga mencetuskan
terbentuknya tumor (Burton et al., 2007). Infeksi dan pelepasan
sitokin proinflamasi akibat inflamasi kronis pada prostat

menyebabkan terbentuknya lesi berupa proliferative inflammatory
atrophy (PIA), dimana terjadi fokal atropi jaringan prostat disertai
dengan proliferasi jaringan epitel dan infiltrasi sel-sel radang
(Burton et al., 2010; Gleave et al., 2010; Patel et al., 2013).
Pada penelitian ini tidak terbukti Ob-Ab dengan kadar CRP
dan IL-6 yang tinggi lebih berisiko mengalami hiperplasia prostat
jika dibandingkan dengan Ob-Ab tanpa hiperplasia prostat.
6.4 Resistensi Insulin Meningkatkan Risiko Hiperplasia
Prostat Melalui Peningkatan IGF-1 Pada Ob-Ab
Pada penelitian ini didapatkan jika HOMA-IR dan Kadar IGF-
1 yang tinggi sama-sama berperan meningkatkan risiko hiperplasia
prostat pada obesitas abdominal. Dari analisis multivariate regresi
logistik, dimana peran resistensi insulin yang dinilai dari HOMA-
IR dan IGF-1 yang tinggi pengaruhnya diperhitungkan secara
bersama-sama didapatkan HOMA-IR yang tinggi meningkatkan
risiko hiperplasia prostat dengan OR=4,18 (IK: 1,15-15,00),
p=0,03; IGF-1 yang tinggi meningkatkan risiko hiperplasia prostat
dengan OR=4,93 (IK: 1,76-13,28), p=0,002. Apabila dianalisis
lebih lanjut peran resistensi insulin (HOMA-IR) dalam
memprediksi dan meningkatkan risiko kejadian hiperplasia prostat
pada Ob-Ab maka didapatkan y=-0,89+1,43(HOMA-
IR)+1,59(IGF-1). Probabilitas untuk kejadian hiperplasia prostat
adalah p=1/(1+e-y
), dimana e adalah bilangan natural bernilai 2,7.
HOMA-IR bernilai 1 bila kadarnya>2,7, IGF-1 bernilai 1 bila
kadarnya >150ng/mL. Semua variabel tersebut bernilai 0 pada
persamaan y bila nilainya <nilai cut-off point tersebut. Sehingga
apabila didapatkan kadar IGF-1>150ng/mL dan HOMA-IR>2,7
maka probabilitas untuk mengalami hiperplasia prostat adalah
89%.
Dari hasil penelitian ini terbukti resistensi insulin yang dinilai
dari HOMA-IR dan kadar IGF-1 yang tinggi bersama-sama
berperan sebagai faktor risiko hiperplasia prostat pada obesitas
abdominal. Hasil penelitian ini didukung oleh beberapa hipotesis
beberapa terori yang berhubungan, yang kami coba analisis dan

sintesis mengingat belum ada penelitian yang meneliti mengenai
resistensi insulin dan IGF-1 dalam meningkatkan risiko hiperplasia
prostat pada Ob-Ab. Penelitian-penelitian yang berhubungan
dengan obesitas, resistensi insulin, IGF-1 sudah dipaparkan pada
Sub Bab 6.1, 6.2, dan 6.3.
Peran sentral Ob-Ab dalam menimbulkan resistensi insulin
akan meningkatkan kadar insulin yang berhubungan dengan efek
mitogenik dan growth promoting effect secara sigifikan. Insulin
sendiri diketahui mampu berperan sebagai growth factor yang
meningkatkan risiko pembesaran prostat yang secara garis besar
melalui aktivasi sistem IGF, sehingga meningkatkan produksi dan
bioavalibilitas IGF-1 (Giovannuci, 2003).
Ob-Ab berhubungan dengan peningkatan respon IGF-1
terhadap GH dan peningkatan protein pengikat GH (Gleeson et al.,
2005). Peningkatan ekspresi reseptor GH dapat menjelaskan
penurunan supresi terhadap kadar IGF-1. Kadar IGF-1 sebanding
dengan akumulasi lemak abdomen pada Ob-Ab (Lukanova et al.,
2003). Suatu teori yang masih dalam batasan hipotesis
menjelaskan jika peningkatan kadar insulin sebagai akibat
resistensi insulin akan menurunkan IGFBP-1 sehingga akan
meningkatkan bioavailibilitas IGF-1 bebas dalam darah. Sehingga
kadar IGFBP-1 dapat disimpulkan berhubungan terbalik dengan
kadar insulin dan jumlah lemak intraabdomen.
Pana penelitian kami, dari analisis jalur (Path Analysis)
ditemukan pola hubungan yang menggambarkan kerangka
patofisiologi resistensi insulin (HOMA-IR), IGF-1 terhadap
hiperplasia prostat. Terdapat hubungan yang signifikan dan erat
antara HOMA-IR dengan hiperplasia prostat (CR=3,64; p<0,001,
efek total 44%, efek langsung 31% serta efek tidak langsung 13%).
Dari analisis ini juga didapatkan hubungan yang signifikan antara
HOMA-IR dengan IGF-1 (CR=4,61; p<0,001, efek langsung dan
efek langsung yang sama besarnya yaitu 379%). Terdapat juga
hubungan langsung yang sangat jelas antara IGF-1 terhadap
hiperplasia prostat (CR=3,19; p=0,001, dengan efek langsung 3%).
Dengan demikian dapat dikatakan bahwa HOMA-IR dan IGF-
1 memiliki efek yang signifikan terhadap hiperplasia prostat dan

hubungan tidak langsung antara HOMA-IR terhadap hiperplasia
prostat melalui peningkatan IGF-1.
Hasil penelitian ini konsisten dengan apa yang didapatkan
oleh Nam et al., pada studi ini ditemukan jika kadar insulin puasa
pada sampel dengan obesitas lebih tinggi bermakna jika
dibandingkan dengan yang tanpa obesitas (Nam et al., 1997). Pada
subanalisis didapatkan hubungan postif bermakna antara IGF-1
bebas dengan insulin puasa (r=0,058, p=0,001).

Insulin dan IGF bekerja melalui dua cara yaitu bekerja
ditingkat sel sebagai growth factor dan sebagai hormon yang
mengatur pertumbuhan dan metabolisme energi. IGF mempunyai
peran yang sangat penting dalam kehidupan seluler, mengatur
pertumbuhan dan perkembangan sel, mengaktifasi proliferasi,
diferensiasi dan transformasi sel, serta menghambat apoptosis
(Baserga et al., 2003; Renehan et l., 2006; McKee et al., 2009;

Lima et al., 2010). Secara struktur IGF-1R mirip dengan resptor
insulin, terdapat 50% homolog antara 2 reseptor tersebut. Kondisi
ini memungkinkan ikatan silang antara IGF-1 dan Insulin pada
reseptor tersebut, sehingga selain peran metabolik dari insulin,
teori ini memungkinkan insulin dapat memberikan efek
antiapoptosis dan efek proliperatif (Giovanna et al., 2009).
Insulin mensupresi beberapa IGFBPs dan menstimulasi
produksi IGFs sehingga meningkatkan bioavailabilitas IGFs.
Insulin memiliki struktur yang mirip dengan IGF-1 dan berikatan
dengan reseptor IGF-1 sehingga dapat mengaktivasi berbagai jalur
pensinyalan yang kompleks yang berhubungan dengan proliferasi
sel-sel pada prostat, atau melalui jalur alternatif yang dapat
menurunkan IGF-1BP sehingga meningkatkan bioavalibilitas IGF-
1 (Sarma et al., 2009).
6.5 Temuan Baru
Dari penelitian yang telah kami lakukan, maka dapat
diuraikan kebaruan dari penelitian ini adalah:
1. Resistensi insulin mempunyai peranan pada patogenesis
hiperplasia prostat pada Ob-Ab.
2. Peran resistensi insulin dalam patogenesis hiperplasia
prostat pada Ob-Ab mungkin diperantarai oleh
peningkatan IGF-1.
7. Simpulan Dan Saran
7.1 Simpulan
Dari hasil-hasil yang ditemukan dari penelitian ini dan
pembahasannya diajukan simpulan sebagai berikut:
1. Obesitas dengan HOMA-IR tinggi lebih berisiko terkena
hiperplasia prostat dibandingkan dengan HOMA-IR rendah.
2. Obesitas abdominal dengan kadar IGF-1 tinggi lebih berisiko
terkena hiperplasia prostat dibandingkan dengan kadar IGF-1
rendah.

3. Resistensi Insulin yang diukur dari nilai HOMA-IR
meningkatkan risiko hiperplasia prostat melalui peningkatan
IGF-1 pada obesitas abdominal.
4. IL-6 dan hsCRP tidak terbukti berperan sebagai faktor risiko
hiperplasia prostat.
7.2 Saran
Dari hasil-hasil penelitian ini dapat diajukan beberapa saran
sebagai berikut:
1. Resistensi insulin yang dinilai dari indeks HOMA-IR dan
kadar IGF-1 dapat dipertimbangkan sebagai faktor yang dapat
dipakai untuk memperkirakan risiko terjadinya hiperplasia
prostat pada obesitas abdominal. Perlu dilakukan penelitian
lebih lanjut yang meneliti ekspresi IGF-1
2. di jaringan prostat dan reseptor IGF-1 untuk menguji lebih
lanjut konsistensi hubungan dan peran sistem IGF dalam
patofisiologihiperplasia prostat pada obesitas abdominal.

INSULINE RESISTANCE WAS INCREACED RISK
FACTOR FOR PROSTAT HYPERPLASIA VIA INSULIN
LIKE GROWTH FACTOR-1 IN ABDOMINAL OBESITY
Pandemic of obesity, especially abdominal obesity (Ab-Ob),
recently become serious threat to human being in the world. Ab-
Ob is often acquired with other symptoms and risk factors that
considered as cardiovascular disease (CVD) risk factors which is
known as metabolic syndrome (MS) (Carr et al., 2004; IDF, 2013).
MS was a risk factors constellation consist of insulin resistance,
Ab-Ob, hypertension, glucose tolerance impairment (prediabetes),
and dyslipidemia that considered as an initial phase on the natural
history of diabetes mellitus (DM) and CVD (Leahy, 2005; Grundy
et al., 2008; Gorbanchyski et al., 2010). On the natural history of
MS as a risk factor of metabolic disorder and CVD, Ab-Ob
occupied the very basic role (Carr et al., 2004; Grundy et al.,
2008).
Since 1980, obesity pandemic became a fact that have serious
attention from WHO. On 2008, the newest data from WHO
showed that more than 1,4 billion world population have an
overweight condition, 200 million male and 300 million female
with obesity (WHO, 2014). More than 10% world population was
in obesity condition. In United State, 30% adult population was in
overweight condition (BMI 25-29,9 kg/m2) and 32% with obesity
(BMI 30 kg/m2) (Ogden et al., 2006). On the age more than 60
years old, the prevalence even reached 40%. In Asia, prevalence of
MS was obtained highest in India due to high prevalence of Ab-
Ob, reaching 1.091 population with 8% prevalence on male and
18% on female (Johnson, 2013).
Prevalence of DM is also keep increasing from year to year
along with increase prevalence of MS and obesity, especially Ab-
Ob. In 2012, International Diabetes Federation (IDF) was
approximating 371 million world people (8,3%) suffer from DM

(IDF, 2012). From that number, half was becoming undiagnosed
cases. Indonesia occupied the 7th rank of 10 countries with the
largest number of DM in the world (IDF, 2012). The number of
DM patient was estimated 7,3 million population on 2011 (IDF,
2011). On 2012, the number of DM patient increased to 7,6 million
(IDF, 2012). This number is predicted increasing continuously
until reach 11,8 million population on 2030. At that time,
Indonesia is predicted on the 9th rank countries with the largest
number of DM patient in the world. The result of RISKESDAS
from Health Minister on 2007 show prevalence of DM in
Indonesia reaching 5,7% (Depkes, 2007).
Complication of MS and DM is started early before diagnosis
enforced. About 50% patient on the time of diagnosis is already
having chronic complication, 21% among them have retinopathy,
18% with abnormal ECG, and 14% with leg blood flow
disturbance, and the rest is associated with gastrointestinal
malignancy and urologic malignancy, especially male (IDF 2012;
WHO 2014). That DM complication reduce the patient
productivity and life expectancy until 15 years. Four point eight
million people in the world died because of DM due to MS on
2012. Mortality rate of diabetes in Indonesia is quite high, about
155.465 Indonesian population died because of DM (IDF, 2012).
Beside as a risk factor constellation that cause DM and CVD,
MS or each components, especially Ab-Ob and insulin resistance,
from some epidemiological study, is correlated with some
degenerative disease, such as prostate hyperplasia (Gorbanchyski
et al., 2010; WHO, 2014). On male, prostate hyperplasia is one of
three most obtained degenerative disease along with increase of
age (other than hypogonad and erection dysfunction) that related to
low grade chronic inflammation. Fifteen percent male population
aged 40 years old were found with prostate disorder and prostatitis,
20-40% with Benign Prostate Hyperplasia (BPH). On 2007, there
were 218.890 new cases of BPH in United State with mortality rate
reaching 27.050 and the incidence increase continuously reaching
230.000 new cases on 2010 (Braun et al., 2011).

Insulin resistance is defined as resistance to insulin metabolic
effect that will cause tissue insensitivity to insulin (Hawkins &
Rossetti, 2005). Insulin metabolic effect include inhibition of
endogen glucose production, stimulation effect on glucose uptake
and tissue glycogen synthesis, and inhibit fat dispersion on adipose
tissue (Ghani, 2006). Without any defect on beta cell pancreas
function, individu will compensate insulin resistance with
increased number of insulin secretion (hyperinsulinemia)
(Masharani et al., 2004).
Ab-Ob is related to insulin resistance as the cause of
hyperinsulinemia (Carr & Brunzell, 2004). Ab-Ob is also related to
low grade chronic systemic inflammation due to increase
proinflammation cytokin such as CRP and protrombotik state
along with increased level of plasminogen activator inhibitor-1
(PAI-1) and oxidative stress (Kahn et al., 2005).
Pathogenesis model of this insulin resistance is integrated
between genetic, obesity, and environmental factor (Masharani et
al., 2004; Hawkins & Rossetti, 2005; Kahn et al., 2005). Genetic
mutation affect the insulin action and insulin secretion defect.
Insulin resistance on obesity is related to sirculation factors that
result from adipocyte such as TNF-α (Tumor Necrotizing Faktor
alfa), FFA (Free Fatty Acid), and leptin. However, the most
dominat is FFA (Evans et al., 2002). Intraabdominal adipose tissue
on abdominal obesity will release excess FFA that can induce
insulin resistance because it will compate with glucose as an
energy source to be oxidized in periferal tissue. In molecular, FFA
can activate protein kinase C (PKC), nuclear factor kappa Beta
(NF-қB), mitogen activated protein kinase (p38MAPK), NH2-
termial jun kinase/Stress activated protein kinase (JNK/SAPK),
that increase serine/threonine phosphorylation on IR or the
substrate as a cause of decrease receptor sensitivity to insulin so
that become compensated with hyperinsulinemia condition. This
signaling pathways is also have a rule in increasing production of
reactive oxygen species (ROS) which is in the end will placed MS
patient, which already have DM, into oxidative stress and

proinflammation condition, especially low grade chronic
inflammation (Evans et al., 2002; Opie, 2007; Grundy, 2008).
Low grade chronic inflammation on MS is happen due to
increase macrophage infiltration to adipose tissue which is marked
with increase proinflammation cytokine such as TNF-, IL-6, and
CRP, have a role in insulin resistance pathogenesis and also
considered have a role in prostate hyperplasia incidence (Hsing et
al., 2007). Inflammation on MS which is related to increase
prostate hyperplasia incidence is hypothezised through: (1)
increase proinflammatory mediator such as cytokin and ROS; (2)
increase oncogen ekspression such as COX-2 and MMP; (3)
increase proinflammation gene transcription factor such as NF-kB,
STAT3 dan HIF- (Bruton et al., 2010; Ramos, 2013). If chronic
inflammation process, ROS, process series that have been
mentioned above about prostate as target organ can cause focal
atrophy of prostate tissue which is accompanied by epitel tissue
proliferation and inflammatory cells infiltration named as PIA
(prolifereative inflammatory atrophy). PIA will transform become
high grade prostate hyperplasia (PIN) (Lehrer 2005; Patel et al.,
2013). Tissue damage and chronic healing process series as
repeated compensation of healing cell damage also will accelerate
BPH nodul occurance (Briganti et al., 2009; Nunzio et al., 2011).
Secondary hyperinsulinemia condition consequence of insulin
resistence in MS also related prostate hypertrophy and clinical
manifestation of Lower urinary tract symptoms (LUTS).
Hyperinsulinemia related to increase of sympathetic nerve activity
and will increase prostate sympathetic tone so that can worsened
LUTS symptom that related to prostate hyperplasia.
Hyperglycaemia alone will increase cytosolic-free calcium in
smooth muscle cells and neuronal tissue will increase sympathetic
nerve activation.

3. Consept, Theoritical Framework, and Research
Hypothesis
3.1 Consept Framework
MS was a risk factor constelation consist of insulin resistance,
abdominal obesity, hypertension, glucose tolerance impairment
(prediabetic), and dyslipidemia as an initial phase on natural
history of a DM and CVD that caused patient into proinflammation
dan prothrombotic condition. Beside as a risk factor that caused
DM and CVD, MS or each components especially Ab-Ob and
insulin resistance from several epidemiological study being related
with high incidence of degenerative disease in urology such as
prostat hyperplasia.
Chronic inflammation with low grade characteristic that
happen on Ab-Ob because increasing macrophage infiltration to
adipose tissue that marked with elevation proinflammation
cytokines such as TNF-, IL-6 and CRP beside have a role in
insulin resistance pathogenesis also in hyperplasia prostate
incidence. Inflammation on Ab-Ob related to increasing prostate
hyperplasia incidence are being hypotesized through: (1) increase
proinflammation mediator such as cytokine and ROS; (2) increase
oncogen expression such as COX-2 and MMP; (3) increase
proinflammation gene transcription factor NF-kB, STAT3 and
HIF-. In chronic inflammation, ROS and process series that have
been mentioned above about prostate as target organ can cause
fokal atophy of prostate tissue which is accompanied by epithelial
tissue proliferation and inflammatory cells infiltration name as PIA
(proliferative inflammatory atrophy). PIA will transformed
become high grade prostateic hyperplasia (PIN). Tissue damage

healing cell damage which accelerate BPH nodul occurance.
Hyperinsulinemia will increase the activity of insulin growth
factor-1 axis (IGF-1 axis), whereas hyperinsulinemia will decrease
insulin growth factor binding protein 1 and 2 (IGFBP-1 and 2),
that have function to bind IGFs and decrease its biological activity
on IGF-1 Receptor (IGF-1R) so that will increase free IGFs that

can increase proliferation and inhibit cell apoptosis. IGF-1 or 2 on
IGF-1r binding will activate thyrosine kinase pathway. Shc
phosphorylation will activating MAPK which related with
proliferation and hyperplasia including elevation malignancy risk.
This thing will accelerate cellular hyperplasia in correlation with
metabolic abnormalities, such as prostate hyperplasia.

4. Research Method
4.1 Research Design
This research uses matched-paired case control study. Case group
is abdominal obesity patient with prostate hyperplasia and control
group is abdominal obesity without prostate hyperplasia.

4.2 Data Source Determination
4.2.1 Research Population
Target population is all Ab-Ob patient with or without prostate
hyperplasia. Accessible population is all patient with Ab-Ob and
prostate hyperplasia treated at Sanglah General Hospital Denpasar
dan Wangaya General Hospital. Intended sample was chosen from
accessible population, after fulfill inclusion and exclusion criteria.
Actual study subjects is sample that willing to join the research by
fill in informed consent form.
4.2.2 Inclusion criteria
Male with Ab-Ob and prostate hyperplasia age > 40 (as cases) and
as control (same age) and willing to join the research until it’s
done.

Patient which fulfill the condition until all the sample filled. Case
with MS and prostate hyperplasia chosen from the patient which
already diagnosed with MS and prostate hyperplasia in Sanglah
Hospital (prostate hyperplasia confirmed by urology department).
Control chosen appropriately with characteristic from each sample
(healthy people without MS and prostate hyperplasia with same

Sanglah General Hospital, on the other hand non-routine like IGF-
1 conduct in Prodia lab of research and development office,
Jakarta. This research start from March 2015 until August 2015.

4.7 Data Analysis
1. Descriptive Statistic
For numeric data descriptive analysis has been done such as
mean±SD (for normal distribution data), median (minimum-
maximum) (for not normal distribution data), and nominal data
presented in frequent data form.
2. Normality Test
For numeric data Kolmogorov Smirnov normality test has
been done, if the data was not in normal distribution, the
hypothesis test will be done with assumption the data in normal
distribution with data transformation through logistic
transformation. The data was normal if p>0,05.
3. Comparison Test
For paired mean group test, before the comparison test being
done, homogeneity group varians test with Levene test.
Comparison test was done with statistical test based on
homogeneity data from Levene test result. Mean test between each
group will be done with parametric test if the data in normal
distribution and the variant is numerical. Paired student t test
conduct hypothetical test to know mean difference between 2
groups. If after numerical data transformation has been done the
result was not normal distribution, then hypothetical test will be
done with non-parametric analysis (Mann-Whitney), with
presenting median (interquartile) data. Chi-Square analysis also
done in this bivariate test to look relationship pattern and OR
magnitude on each independent variable.
4. Multivariate Test
Multivariate Test can be used to look relationship between
one or more dependent variable with one or more independent
variable. This test also used to look contributive relationship from
various independent variable to dependent variable or to control
confounding variable, so that relationship between independent
variable hypotheses with dependent variable can be seen clearly.

Logistic regression analysis has been done to control confounding
variable, so that the impact of independent variable can be seen
between dependent variable with nominal scale. Presented
Estimated Odd Ratio (OR) with Confidence Interval 95%.
5. Path Analysis
To look direct relationship between HOMA-IR, IGF-1, IL-6
and hsCRP to prostate hyperplasia and indirect relationship
between HOMA-IR to prostate hyperplasia through IGF-1
elevation.
6. Significancy level (α) of this research assigned to probability
value (p) less than 0,05. Statistical test with software SPSS for
Mac 21 version and AMOS 23 version.
5. Result
5.1 Basic Characteristics of Research Subjects
A total 80 samples of male included in this study. Forty samples as
the case group, with prostate hyperplasia and 40 samples as a
control, with mean age of 62 years old. After the normality test
data by Kolmogorov-Smirnov test, and after the logarithm
transformation, a several variables in this study has an abnormal
distribution, like hs-CRP, IL-6, fasting insulin, fasting blood
glucose and 2-hour PP, HOMA-IR, TG, SC, body weight, BMI,
and waist circumference (p < 0.05).
Baseline characteristics of Research Subjects (Demographic
Characteristics and Nutrition Status) from the group case (Ob-Ab
with Prostate Hyperplasia) and control group (Ob-Ab without
prostate hyperplasia). Basic characteristics data of this study are
presented in Table 5.1.

Data with normal distribution, concentration measurement
data is presented in the meanSD, while an abnormal distributed
data presented in median (minimum-maximum). Mean different
test (independent t test for normally distributed variables, Man-
Whitney for abnormal distribution variables) showed that some
variables showed significant differences between cases and
controls. Fasting glucose (Z= -3.325, p= 0.001), fasting insulin (Z=
-2.15, p= 0.034), HOMA-IR (Z = -2.843, p = 0.004), and IGF-1 (t
= 3 , 27 , p = 0.002) and significantly higher in cases group (Ob-
Ab with prostate hyperplasia) compared to controls (Ob - Ab
without prostate hyperplasia). On the other hand, hsCRP and IL-6
did not differ significantly between the two groups. So from the
bivariate analysis only insulin resistance and IGF-1 were shown to
be higher and meaningful in the Ob-Ab with prostate hyperplasia
when compared with the Ob-Ab without prostate hyperplasia.
5.2 Chi-Square Bivariate Analysis Insulin Resistance (HOMA-
IR), IGF-1, Inflamation (IL-6, hsCRP) and Prostate

To assess the role of insulin resistance (HOMA-IR), IGF-1 as a
risk factor for prostate hyperplasia performed logistic regression
analysis to find the value OR. Cut-off point based on the study and
the literature used to divide the numeric variables into
dichotomous variables. Normal IGF-1 levels to the normal
population, and for over 40 years was 150 ng/mL (Bravermen et
al., 2013). Normal IGF-1 (the lowest value of 150 ng/mL) and
IGF-1 high (> 150 ng/mL). To cut-off point value of HOMA-IR,
insulin resistance in healthy populations without diabetes was 2.7
(Komshian et al., 2000). So the value of HOMA-IR in this study
were divided into insulin resistance ( 2,7) and without insulin
resistance (low - value 2) . On the other hand for a marker of
inflammation, the value of CRP and IL-6 is considered the upper
limit of normal in the population at risk for developing insulin
resistance are respectively 1.32 mg / L and 12.56 pg / mL (Deepa
et al., 2006). But in this study IL-6 values of all samples either in
cases or controls are within the normal range (0.15 to 5.46 pg /
mL), so that the cut-off point used is from the median of the results
of this study are 0.875 pg / mL. The result of bivariate analysis
with chi-square test are presented in Table 5.2 while the results of
logistic regression that describes the role of HOMA-IR and IGF-1
are high risk factors for prostate hyperplasia in Ob-Ab is presented
in Table 5.3.

From the test results obtained bivariate chi-square significant
association between insulin resistance (HOMA-IR) and IGF-1 in
prostate hyperplasia. When viewed one by one in bivariate,
HOMA-IR and IGF-1 respectively increase the risk of prostate
hyperplasia. HOMA-IR increased the risk of prostate hyperplasia
with OR = 1.94 (CI 1.30 to 2.89) and statistically significant with p
= 0.005. IGF-1 increases the risk of prostate hyperplasia with OR =
2.14 (CI 1.38 to 3.3 ), p = 0.001.
From the results of this analysis also found that the
inflammatory variables such as IL-6 and hsCRP was not
significantly associated with prostate hyperplasia. hsCRP with OR
= 0.9 ; p = 0.653, while IL-6 value OR = 1.1105 , with p = 0.655.
However, there is a tendency of IL-6 enhances prostate
hyperplasia, although not statistically significant.
5.3 Multivariate Logistic Regression Analysis HOMA-IR and
IGF-1 toward Prostate Hyperplasia in Ob-Ab.
In multivariate analysis, logistic regression with backward method,
when the role of HOMA-IR, IGF-1 calculated simultaneously,
HOMA-IR (Insulin Resistance) and IGF-1 still significantly raise
risk for prostate hyperplasia.

Insulin Resistance (HOMA-IR) and IGF have significant
influence towards the incidence of prostate hyperplasia in patient
with abdominal obesity (table 5.3) with OR and p-value OR=4,18;
p=0,03; IK 1,15-15,00 and OR=4,93; IK 1,76-13,78; p=0,002. If
we analyze more thoroughly the role of insulin resistance (HOMA-
IR) in predicting and elevating the risk of prostate hyperplasia in
Ab-Ob we gained y=-0,89+1,43(HOMA-IR)+1,59(IGF-1). The
probability of prostate hyperplasia was p=1/(1+e-y
),where are
natural number with value 2,7. HOMA-IR value is 1 if the
concentration >2,7. IGF-1 worth is 1 if the concentration is
>150ng/mL. All variable value was 0 in y equation if the value less
than cut off point. If the result of IGF-1>150ng/mL and HOMA-
IR>2,7 than the probability of prostate hyperplasia was 89%.

5.4 Path Analysis Relationship of HOMA-IR, IGF-1, and
Prostate Hyperplasia
To see the causal effect relationship of HOMA-IR, IGF-1 to
prostate hyperplasia we use path analysis. In this analysis external
variable was HOMA-IR, IGF-1, IL-6 and hsCRP. And the internal
variable was prostate hyperplasia. Based on the literature review
and analysis results like the above, then we made models and
output analysis and present it in Figure 5.1.

In structural phase model we obtained direct effect to
BPH/LUTS: HOMA-IR toward BPH/LUTS was 0,31 (31%), IGF-
1 toward BPH/LUTS was 0,03 (3%), IL-6 toward BPH/LUTS was
0,10 (10%) and hsCRP toward BPH/LUTS was 0,05 (5%). On the
other hand direct effect of HOMA-IR toward other variable: effect
HOMA-IR toward IGF-1, IL-6, hsCRP consecutively was 3,79
(379%), 0,005 (0,5%) and 0,015 (1,5%). From this structural
model it appears that HOMA-IR has the most powerfull effect
toward IGF-1, while the most powerful variable that influence
BPH/LUTS was HOMA-IR.
Relationships between constructs was presented in Table
5.4 once analyzed with AMOS. The magnitude of each
independent variable toward dependent variable represented by
critical ratio (CR). CR Value is obtained from estimate valued
divided by standard error (SE). The higher the value of CR hence
the effect is more significant.

From the analysis of relationship between two variables
constructs in Table 5.4, we could interpreted: there is a significant
relationship between HOMA-IR with IGF-1 (CR=4,61; p<0,001),
HOMA-IR with prostate hyperplasia (CR=3,64; p<0,001), IGF-1
with hyperplasia prostate (CR=3,19; p=0,001). On the other hand,
we found no significant relationship between IL-6 and hsCRP
toward prostate hyperplasia.
Based on the value of Standardize weight regression we can
see how strong the relationship between variables constructs.
Loading factor HOMA-IR and IGF-1 toward prostate hyperplasia
consecutively 0,37 and 0,32. This means HOMA-IR and IGF-1 can

explain the incident of BPH, there is a close relationship between
HOMA-IR and IGF-1 toward prostate hyperplasia. On the other
hand loading factor HOMA-IR toward IGF-1 was 0,46. This
suggests the close relationship between HOMA-IR and IGF-1.

From table 5.5 we can see HOMA-IR have the most powerful
total and direct effect against prostate hyperplasia. On the other
hand in Table 5.6 clearly show the most powerful direct and total
effect HOMA-IR to IGF-1, followed by HOMA-IR toward
prostate hyperplasia. In Table 5.5 we can also see HOMA-IR still
contributes an indirect effect on prostate hyperplasia, while IGF-1
only have direct effect toward hyperplasia prostate.

shows that there is a close and significant relationship between
HOMA-IR with prostate hyperplasia (CR=3,64; p<0,001, total
effect 44%, direct effect 31% and indirect 13%). From this analysis
we also found a significant relationship between HOMA-IR with
IGF-1 (CR=4,61; p<0,001, immediate effect and the indirect effect
is the same magnitude 379%). There is also a very clear direct
relationship between IGF-1 against prostate hyperplasia (CR=3,19;
p=0,001, direct effect 3%).

Thus it can be said that HOMA-IR and IGF-1 has significant
effects on prostate hyperplasia. From the structural analysis model
is presented in Figure 5.1 and a path analysis above, we can see a
direct relationship between HOMA-IR toward hyperplasia prostate
through increased of IGF-1.
6. Discussion
6.1 Insulin Resistance and Prostate Hyperplasia on Ab-Ob
This Research get result if HOMA-IR more higher and significant
on Ab-Ob group with prostate hyperplasia compared with group
without prostate hyperplasia (1,88 vs 1,19, p=0,002). High HOMA
IR >2,7 or in resistance insulin condition increase prostate
hyperplasia risk with OR=1,94 (IK 1,30-2,89), and significant
(p=0,005). This research is consistent with what Zhang et al.
achieved, through this epidemiology study which involve 401
geriatric male sample also achieved if male with MS has prostate
volume bigger and significant (p=0,000) if compared with without
MS (Zhang,et al.,2014). Further sub analysis in this research get
significant positive correlation between prostate volume and BMI
(P=0,000), HOMA-IR (P=0,003) and negative correlation with
HDL level (p=0,000). On linear regression multivariate analysis
prostate volume significant correlated with HOMA-IR (p=0,015).
Kim et al., which examine 212 BPH patient without DM get
significant positive correlation between fasting glucose level with
prostate size (r=0,186; p=0,007) (Kim et al., 2011). This Study also
examine BMI, fasting glucose level and insulin resistance towards
prostate size which assessed form transrectal USG. But on this
research didn’t get significant correlation between insulin
resistance (HOMA-IR) and fasting glucose level and prostate size.
MS prevalence on community with prostate hyperplasia also
higher if compared with without MS (60,3% vs 46,1%, p=0,018)
(Ryl et al., 2015). This research uses case control design with
healthy people control. Further in this research also proved
significant correlation between BPH with several MS component
like glucose level, total cholesterol, LDL, HDL and systolic and
diastolic blood pressure. But on this research not obtained

significant correlation between prostate hyperplasia with waist
circumference, weight, and TG concentration.
Ozden et al., in a more detail examined correlation between
MS with increase of prostate size every year, which involve 78
patient with prostate hyperplasia symptom divided into group with
MS and without MS. (Ozden et al., 2007). On MS group with
prostate hyperplasia median value weight, BMI, TG and blood
glucose which higher if compared with BPH patient without MS.
On MS group with prostate hyperplasia in the end of research
found increase of total prostate volume 1 ml every year while BPH
group without MS only 0,93 ml, with significant difference in
statistic (p<0,05). From this study pictured insulin resistance
central role on MS in its correlation with prostate hyperplasia.
Obesity consistency as prostate hyperplasia risk factor also
proved on research by Lee et al., which examined Obesity effect
which measured by BMI parameter to prostate hyperplasia (Lee et
al., 2006). In this research included 146 male patient age >40 years
old which not experience DM. Prostate size examination with USG
transrectal. Research subject divided into 3 group consist of normal
(BMI 22,9 kg/m2), overweight (23-24,9 kg/m
2) and obesity (>25
kg/m2) and two group based on waist circumference (>90cm) and
(<90 cm) Prostate volume obtained the higher and significant on
obesity group (p=0,03) and on abdominal obesity (p=0,002). After
doing adjusted to some confounding variable, abdominal obesity
become primary risk factor incidence of prostate hyperplasia
(prostate volume >20 mL) (OR=3,7,p=0,037).
Hyperinsulinemia which inducted by high carbohydrate and
fat diet, can caused increased of cell activation for excessive
proliferation. This research show hyperinsulinemia correlated with
increase of prostate size about 45% from rat experiment. This
result can give picture that obesity with hyperinsulinemia have
strong relation with prostate hyperplasia incident (Renehan et al,.
2008, Allotet al., 2013)
Hyperinsulinemia and insulin resistance in other way can
caused increased sympathetic neuron system activity, by increasing
cytosolic-free calcium on smooth muscle cell and neural network

so that will increased smooth muscle prostate tone so that will
increase LUTS symptom and prostate hyperplasia (Sama et al,.
2009). So abdominal obesity with high HOMA-IR or insulin
resistance have more experience risk to prostate hyperplasia
compared with abdominal obesity with low HOMA-IR or without
insulin resistance.
This research was supported direct and indirect with results
from other study that we explain above so Ab-Ob with insulin
resistance have a high risk to undergo prostate hyperplasia if
compare to abdominal obesity without insulin resistance. If we see
from prostate hyperplasia, clinical aspect that we can synthesize
from patient with IPSS score >8, LUTS symptom with insulin
resistance increase risk of prostate hyperplasia case then in Ab-Ob
with insulin resistance improvement, effort to decrease waist
circumference can suggest earlier so that risk of prostate
hyperplasia can be decrease.

Insulin resistance that can cause hyperinsulinemia in Ab-Ob
can increase risk of prostate hyperplasia if we synthesize from
concept and theory about IGF-1 receptor considered that insulin
have a homolog characteristic with IGF-1, or via indirect pathway
through decrease of IGF-1BP so that can increase free IGF-1 level.
Verification about this further mechanism will be explained in
chapter 6.2 and 6.4.
6.2 High level of IGF-1 and Prostate Hyperplasia on Ab-Ob
In this research we find that high level of IGF-1 and significant on
Ab-Ob group with Prostate hyperplasia if compare with Ab-Ob
group without prostate hyperplasia (165,4864,62 vs
127,5334,63, p=0,002). Also we find if high level IGF-1 (>150
ng/mL) increase risk significantly with OR=2,139 (CI=1,34-3,3)
and p value = 0,001. This result consistent with other study prove
that high level of IGF-1 in patient with prostate hyperplasia and
obesity if compare with group without prostate hyperplasia.
IGF-1 level significantly correlate with prostate volume in
case of prostate hyperplasia (r=0,596, p=0,007), although there is
no significant correlation between prostate cancer (Ayati et al.,
2012). This result describe if there is a mechanism that involve of
IGF-1 within increase prostate cells proliferation but can’t cause
advance cells differentiation. IGF-1 signaling pathway very
important in cells growing and differentiation so that increase level
and expression of IGF-1 correlate to increase risk of hyperplasia.
Animal study by Lefko et al, find that IGF-1 give cells hyperplasia
effect on prostate cells with quite specific mechanism (Lefko et al.,
2008). In this study prove that in transgenic mice (PB-Des),
expression of IGF-1 will increase hyperplasia lesion in prostate,
but this condition never fall to adenocarcinoma.
If we studied and synthesize deeply, in normal prostate cells,
stromal cells produce IGFs, IGFBP-2, IGFBP-3, IGFBP-4
although dominant epithel cells express IGF-1R, IGFBP-2 and
IGFBP-4 also fraction of IGFBP-3 and IGFBP-6 (Russel et al.,
1998). In prostate with proliferation condition, epithelial cells

produce cathepsin D and PSA, which can play role like protease
that can broke bond between IGFs and IGFBPs so that can increase
free IGFs (Lubik et al., 2011). Over expression of IGF-1 can
stimulate cells growth, proliferation and antiapoptotic via paracrine
mechanism or autocrine (Dasgupta et al., 2012).
Insulin suppress any IGFBPs and stimulate to produce IGFs
so that can increase bioavailability of IGFs. Insulin have a similar
structure with IGF-1 and bond to IGF-1 receptor so that can
activated many complex signaling pathway that related to cells
proliferation in prostate or via alternative pathway that can
decrease IGF-1BP so that cant increase bioavailability of IGF-1
(Sarma et al., 2009).
Bond between IGF-1 and insulin with IGF-1R induce
activation of tyrosine kinase pathway intracellular (Burton et al.,
2010). Fosforilation from Shc can activate MAPK pathway that
will stimulate cells growth and proliferation, while IRS
fosforilation will activate PI3K pathway than can inhibit cells
apoptotic and proliferation via stimulation of any transcription
factor like NF-κB (nuclear factor-κB), and mTOR (mammalian
Target Of Rapamycin). In molecular perspectives, activation of
tyrosine kinase pathway have an important role to trigger prostate
hyperplasia event (Rawlands et al., 2012).
If we test with bivariate analysis, prove that if Av-Ob with
high IGF-1 level (>150 ng/mL) have higher risk to undergo
prostate hyperplasia if compare low IGF-1 level in Ob-Ab patient.
6.3 Inflammation (IL-6 dan hsCRP) and Prostate Hyperplasia
on Ab-Ob In relation with inflammation based on theorical synthesize
assumed as central role and intermediate variable that bridging
between insulin resistance with prostate hyperplasia on Ab-Ob, on
this research was not achieve significant difference on IL-6 and
hsCRP level on Ab-Ob group with prostate hyperplasia compared
with group without prostate hyperplasia. However in this research
find there is a tendency of IL-6 and hsCRP as inflammation marker
case group between control group continuously 1,07 (0,25-5,68) vs

0,71 (0,15-3,62) pg/dL and 1,45 (0,3-13,5) vs 1,00 (0,2-9,2) mg/L.
This result may be cause by mean in case group or control group
than have normal IL-6 level. Interleukin and acute phase reactant
levels very fluctuate in plasma so that can’t describe reality
condition. In other hand condition that may be affect sample in this
research was taken operative patient so that cytokines load will be
decrease parallel with decrease of prostate mass after hyperplasia.
Cross sectional study by Saadi et al., find that if hsCRP and
IL-6 the highest level was in BPH group with LUTS if compare
with BPH group alone or with normal people This study involve
56 sample that divided by 3 group, BPH+LUTS group, BPH alone
and healthy people. hsCRP level mean for each group was
10,04±1,58; 5±0,36; 2,8±0,5 mg/L with p<0,001. IL-6 level mean
was 14,56±1,1; 8,82±0,93; 12,6±0,82 with p<0,001 (Saadi et al.,
2012).
Other study by Hung et al., find there is significant correlation
between hsCRP to prostate volume (Hung et al., 2014). This study
was a cross sectional with high sample size that involve 853
sample. Mean of hsCRP level on subject base characteristic was
3,1±0,43 mg/dL, significantly correlated to IPSS score with
correlation coefficient r=0,151, p<0,001.
If we compare our research, we find that a very low level of
IL-6 on Ab-OB group with prostate hyperplasia, even with range
between 0,25-3,72 mg/dL. There is a high influence to
insignificant results of IL-6 and hsCRP that increase risk of BPH
on Ab-OB although conceptually and theoretically inflammation
have a central role in correlation between fibrosis and prostate
hyperplasia.
In this research we only find that there is a tendency of IL-6
and hsCRP higher than case group if compare with control group.
IL-6 and hsCRP level in our research lower than other study above.
Several epidemiologic study show that 25% of malignancy
correlated with chronic inflammation and 15% death case from
cancer caused by inflammation (Hsing, 2007; Burton et al., 2007).
Release of inflammation mediator that chronically happen and
excessive theorically considered cause increase initiation,

promotion and progression of tumor (Lehrer et al., 2005; Bruton et
al., 2007).
On obesity, adipose tissue in chronic inflammation condition,
which have important role to insulin resistance, dyslipidemia
accuracy and also other comorbidity disease such as cardiovascular
disease and cancer. This inflammation is being mark with elevation
CRP level, IL-6, IL-8, MCP-1 and TNFα.
Mechanism that explained how obesity induced chronic
inflammation can increase prostate hyperplasia include: (ii)
elevation proinflammation mediator production such as cytokine,
chemokine, ROI, (ii) increase oncogene expression, COX-2, 5-
LOX, and MMPs, (iii) elevation pro inflammation transcription
factor, such as NF-κB, STAT3, AP-1.
Infiltration from chronic inflammation often happen at
peripheral side of the prostate. Inflammation described with PMN
infiltration, cytokine and chemokine including CRP. CRP also
release from adipose tissue. CRP is acute phase protein that
secreted from liver and unspecific marker for inflammation,
infection, tissue damage. This inflammation mediator will
inducedother proinflammation transcription factor such as NF-κB
and (iNOS). NF-κB was strong inducer from gene that related to
anti apoptosis activity and gene that arrange cell cycle life (cyclin
D1). NF-κB and iNOS are very important role in cellular immune
activity, had antimitogenic and antiapoptosis effect. Activation
NF-κB and iNOS by proinflammtion cytokine will induce tumour
cell initiation and progressivity.
Proinflammation cytokine will also induce PMN to produce
ROS and RNS, which next induce DNA mutation and form tumor.
Infection and releasing proinflammation cytokine caused by
chronic inflammation on prostate will cause lesion like PIA,
whereas the focal atrophy on prostate tissue with epithel tissue
proliferation and inflammation cells infiltration.
In this research there is no prove that Ab-Ob with high level
of CRP and IL-6 level have more risk to prostate hyperplasia if
compared with Ab-Ob without prostate hyperplasia.

6.4 Insulin resistence increase risk of Prostate Hyperplasia via
increase of IGF-1 on Ab-Ob
In this research find that if HOMA-IR and IGF-1 level was high
both have role to increase risk of prostate hyperplasia in abdominal
obesity. Logistic regression multivariate analysis show that insulin
resistance assessed from high level of HOMA-IR and IGF-1 state
the high HOMA-IR increase risk of prostate hyperplasia with
OR=4,18 (CI: 1,15-15,00), p=0,03; High level of IGF-1 increase
risk of prostate hyperplasia with OR=4,93 (CI: 1,76-13,28),
p=0,002. If further analysis was done, role of insulin resistance
(HOMA-IR) to predict and increase risk prostate hyperplasia event
on Ab-Ob show y=-0,89+1,43(HOMA-IR)+1,59(IGF-1).
Probability for prostate hyperplasia event was p=1/(1+e-y
), when e
was natural number with value 2.7. HOMA-IR value 1 when level
was >2.7, IGF-1 value 1 when level was >150ng/ml. All variable
value o in y equation when value <cut off points. So that if we get
IGF-1 >150ng/ml and HOMA-IR >2.7 then probability to undergo
prostate hyperplasia is 89%.
Result of this research prove that insulin resistance that
assessed from HOMA-IR and IGF-1 level together have role as
risk factor to prostate hyperplasia on abdominal obesity. This result
was supported by several hypothesis and correlated theory that we
try to analysis and synthesize reckon that there is no research about
insulin resistance and IGF-1 within increase risk of prostate
hyperplasia on Ab-Ob. Another research that related to obesity,
insulin resistance, IGF-1 already explained in chapter 6.1, 6.2 and
6.3
Central role of Ab-Ob within induce insulin resistance will
increase insulin level that related to mitogenic effect and growth
promoting effect significantly. We know insulin have role as
growth factor that increase risk of prostate hypertrophy via
activation of IGF system so that can increase production and
bioavailability of IGF-1 (Giovannuci, 2003).
Ab-Ob relates with increased level of IGF-1 to GH and
increase GH binding protein (Gleeson et al., 2005). Increase
expression of GH receptor can describe suppression decrease to

IGF-1 level. IGF-1 level comparable to abdomen fat accumulation
on Ab-Ob (Lukanova et al., 2003). A theory that still in hypothesis
describe if insulin level increase as a result of insulin resistance
will decrease IGFBP-1 so that will increase bioavailability of free
IGF-1 in the blood. So, IGFBP-1 level can be summarize have
reverse relation to insulin level and intraabdomen fat count.
In this study, from path analysis was found relationship
pattern that describe insulin resistance pathophysiology framework
(HOMA-IR), IGF-1 to prostate hyperplasia. There is a significant
relationship between HOMA-IR with prostate hyperplasia
(CR=3,64; p<0,001, total effect 44%, direct effect 31%, and
indirect effect 13%). From this analysis was also found significant
relationship between HOMA-IR with IGF-1 (CR=4,61; p<0,001,
direct and indirect effect 379%). Also found a direct relationship
between IGF-1 to prostate hyperplasia (CR=3,19; p=0,001, direct
effect 3%).
Therefore can be conclude that HOMA-IR and IGF-1 have
significant effect to prostate hyperplasia and indirect relationship
between HOMA-IR to prostate hyperplasia through increase of
IGF-1.
This research result is consistent with Nam et al. In this study
was found that fasting insulin level on sample with obesity is
significantly higher if compared with sample without obesity (Nam
et al., 1997). On the sub analysis was found positive relationship
between free IGF-1 with fasting insulin (r=0,058, p=0,001).
Insulin and IGF work through 2 mechanism that is work in
cell level as a growth factor and as a hormone that regulate growth
and energy metabolism. IGF has an important role in cellular life,
regulate growth and development of cell, proliferation activation,
cell differentiation and transformation, and inhibit apoptosis
(Baserga et al., 2003; Renehan et l., 2006; McKee et al., 2009;
Lima et al., 2010). Structurally, IGF-1R mimics insulin receptor,
present 50% homolog between that 2 receptor. This condition
possibly make cross bound between IGF-1 and insulin on that
receptor, so that other than metabolic role of insulin, this theory

make insulin can give antiapoptosis effect and proliferative effect
(Giovanna et al., 2009).
Insulin suppress some IGFBPs and stimulate production of
IGFs so can increase bioavailability of IGFs. Insulin have a
structure mimics IGFs and bind with IGF-1 receptor so can
activate some complex signal pathways that related with prostate
cells proliferation, or through alternative pathways that can
decrease IGF-1BP so increase bioavailability of IGF-1 (Sarma et
al., 2009).
6.5 New Finding
From this research, can be explained a novelty:
1. Insulin resistance have a role in prostate hyperplasia
pathogenesis on Ab-Ob.
2. Role of insulin resistance on prostate hyperplasia
pathogenesis on Ab-Ob may mediated by increase IGF-1.
7. Conclusion and Suggestion
7.1 Conclusion
After discussion about result analysis above we can conclude that:
1. Obesity and high HOMA-IR present as high risk factor to
prostate hyperplasia between low HOMA-IR alone.
2. Abdominal obesity with high IGF-1 present as high risk
factor to prostate hyperplasia between low IGF-1 alone.
3. Insulin resistance that count from HOMA-IR increase risk
of prostate hyperplasia via increase of IGF-1 in abdominal
obesity.
4. There is no evidence that IL-6 and hsCRP related to risk
factor of prostate hyperplasia.
7.2 Suggestion
After we conduct this study, we can propose several suggestion:
1. Insulin resistance that we got from HOMA-IR index and
IGF-1 level can be considered as risk factor of prostate
hyperplasia in abdominal obesity.

2. We need to make furthermore study about expression of
IGF-1 in prostate tissue and IGF-1 receptor to verify about
correlation and role of IGF system if pathophysiology of
prostate hyperplasia in abdominal obesity.
